手术室口诀式项目管理预防原位肝移植患者术中获得性压力性损伤的效果

摘要
目的 探讨手术室口诀式项目管理预防原位肝移植患者术中获得性压力性损伤(intraoperative acquired pressure injury,IAPI)的效果.方法 选取 2022 年 1 月-2023 年 10 月于笔者所在医院行肝移植手术的 64 例患者为研究对象.按照入院先后顺序,将 2022 年 1 月-12 月的 32 例患者设为对照组,将 2023 年 1 月-10 月的 32 例患者设为观察组.对照组实施术中 IAPI风险常规管理,观察组采用"一看、二定、三查、四准、五巡"的口诀式项目管理,比较 2 组 IAPI预防措施落实情况及 IAPI发生率.结果 观察组 IAPI 预防措施落实得分高于对照组(t=6.920,P＜0.001),IAPI发生率低于对照组(χ2 =6.093,P=0.014).结论 手术室口诀式项目管理可以促进肝移植患者 IAPI预防措施的落实,降低术中获得性压力性损伤发生率,值得临床推广应用.
Abstract
Objective To explore the effect of prevention intraoperative acquired pressure injury(IAPI)in orthotopic liver transplantation(OLT)recipients based on the formula project management.Methods Totally 64 OLT recipients treated in the researcher's hospital between January 2022 and October 2023 were selected as the subject.The 32 recipients in year 2022 were selected into a control group,while another 32 recipients operated in the following ten months were chosen into an observation group.The control group was given routine management against IAPI,while the observation group prevented the IAPI by the self-made formula one observation,two decisions,three checks,four grants and five patrolling.The implementation of IAPI prevention and incidence of IAPI were compared between the two groups.Results The implementation of IAPI preventive measures was significantly higher than that of the control group(t = 6.920,P＜0.001),while the incidence of IAPI in the observation group was significantly lower than that of the control group(χ2 =6.093,P=0.014).Conclusion The self-made formula management can promote the implementation of IAPI prevention measures and lower the incidence of IAPI.Therefore,it is worthy of promotion in the clinical practice.
