缺血性脑卒中支架置入术后患者运动恐惧影响因素的质性研究

摘要
目的 探讨缺血性脑卒中支架置入术后患者运动恐惧的影响因素,为制定针对性的干预措施提供参考.方法 采取目的抽样法,选取 2023 年 4 月—9 月在武汉市某三级甲等医院神经内科住院的缺血性脑卒中支架置入术后的 14 例患者为研究对象,对其进行半结构式深度访谈,使用内容分析法对访谈资料进行分析、归纳并提炼主题.结果 共提炼运动恐惧内部影响因素和外部影响因素两大主题.内部影响因素包括支架置入术后自我感觉衰弱、对体内支架状态产生过度警觉、对机体不适的敏感性增强、认知偏差引发自我设限、运动自我效能降低 5 个子主题;外部因素包括疾病经济负担、照顾者支持不足、社交疏远、医疗信息缺乏 4 个子主题.结论 缺血性脑卒中支架置入术后患者的运动恐惧受多种因素的共同影响,医护人员应重视此类患者的早期评估和干预,提高患者对支架置入手术的心理调适能力和术后运动康复认知水平,改善社会支持系统,激发患者运动意愿,促进疾病康复.
Abstract
Objective To explore the influencing factors of kinesiophobia in ischemic stroke survivors after stent implantation,so as to provide references for working out targeted exercise rehabilitation scheme.Methods Fourteen ischemic stroke survivors undergoing stent implantation in neurology department of a grade-A tertiary hospital in Wuhan between April and September 2023 were selected and conducted semi-structured in-depth interviews.Then the data were analyzed,summarized and extracted using the content analysis method.Results Two themes of internal and external influencing factors of kinesiophobia were extracted.The former included self-weakening after stent implantation,hyperalertness to the stent,increased sensitivity to body discomfort,self-handicapping caused by cognitive bias,and decreased exercise self-efficacy,while the latter consisted of economic burden of disease,insufficient caregiver support,social distancing and lack of medical information.Conclusion The kinesiophobia of ischemic stroke survivors after stent implantation is caused by various factors.Medical staff should conduct early assessment and intervention,to improve their psychological adjustment,consciousness of postoperative exercise rehabilitation,social support and willingness to exercise,so as to promote their recovery.
