中重度青少年特发性脊柱侧凸患者疾病体验的质性研究

摘要
目的 了解中重度青少年特发性脊柱侧凸患者的疾病体验和现存问题,为制定针对性的干预方案提供参考依据.方法 采用描述性质性研究,根据目的抽样法,选取 2022 年 7 月-2023 年 2 月郑州市某三级甲等医院骨科收治的 13 例青少年特发性脊柱侧凸患者为研究对象,对其进行半结构式深入访谈,并采用 Colaizzi 7 步分析法对访谈资料进行分析、总结归纳以及提炼主题.结果 中重度青少年特发性脊柱侧凸患者的疾病感受和生活体验可归纳为 3 个主题:生活质量下降(睡眠形态紊乱,日常活动能力下降,生活方式被迫改变);消极的心理体验(病耻感较强,情绪消沉,灾难化解释);渴望得到理解支持和专业指导(期望得到他人理解,希望得到社会支持,希望得到医护人员的专业指导).结论 中重度青少年特发性脊柱侧凸患者在生理、心理、社会等方面存在较多问题和需求,医护人员应密切关注青少年特发性脊柱侧凸患者的生理不适和心理情绪,构建有针对性的干预模式和多维度社会支持.
Abstract
Objective To understand the perception and experience of adolescent idiopathic scoliosis,with the aim of providing targeted medical services to reduce their psychological problems.Methods Thirteen adolescents with idiopathic scoliosis and treated in a grade-A tertiary hospital between July 2022 and February 2023 were selected and given a semi-structured interview.Then,the Colaizzi's 7-step analysis method was applied to analyze,summarize and extract the themes.Results Three themes were extracted,including decreased life quality(abnormal sleeping,decreased activities of daily life and reluctant changes in lifestyle),negative psychological experience(shame of being ill,low spirits and catastrophizing interpretation),desire for understanding and social support(expecting personal,social and professional support).Conclusion Adolescent idiopathic scoliosis patients have multiple problems and needs in the physical,psychological and social aspects.Therefore,targeted psychological intervention and multi-dimensional social support should be provided to them.
