摘要
目的 系统评价和整合有关ICU护士道德困境体验的质性研究,为临床制定减轻道德困境的干预策略提供参考.方法 检索Web of Science、Embase、PubMed、EBSCOhost、中国知网、万方、维普、中国生物医学文献数据库中与 ICU护士道德困境相关的质性研究,检索时限为建库至 2023 年 7 月,采用澳大利亚循证卫生保健中心质性研究质量评价标准评价纳入的文献,采用汇集性整合方法进行 Meta整合.结果 共纳入 9 篇研究,提炼出 39 个研究结果,归纳形成 8 个新类别,综合得出 3 个整合结果:造成 ICU护士道德困境的因素、道德困境的感受、道德困境的应对方式.结论 ICU护士道德困境的产生常与多种因素相关,道德困境会对 ICU 护士身心产生伤害,威胁护理安全.建议积极关注 ICU护士的道德困境问题,并建立有针对性的支持系统,帮助护士积极应对道德困境.
Abstract
Objective To systematically evaluate and integrate qualitative research on the experience of moral distress experienced by ICU nurses,so as to provide references for clinical intervention strategies.Methods Qualitative research on moral distress of ICU nurses were searched in PubMed,Web of Science,Embase,EBSCOhost,CNKI,Wanfang,VIP,and China Biology Medicine Disc,with the time setting from the inception until July 2023.The articles selected were evaluated using the quality evaluation criteria of Australian evidence-based health care center,and Meta integration was performed using the pool integration method.Results From 9 studies selected,39 research results were extracted and summarized into 8 new categories,and further integrated into 3 results of the factors contributing to the moral distress of ICU nurses,feelings of moral distress,and ways of coping with moral distress.Conclusion The moral distress of ICU nurses is often related to many factors,and hurt them physically and mentally,affecting nursing safety.It is recommended to pay attention to such problem,and establish targeted support systems to help nurses to deal with it.
