肺癌化疗患者社会隔离体验的质性研究

摘要
目的 探讨肺癌化疗患者社会隔离的真实体验,为制定针对性的干预方案提供依据.方法 采用质性研究方法,于2023年9-11月对河南省人民医院收治的15例肺癌化疗患者进行半结构式访谈,采用内容分析法归纳并提炼主题.结果 归纳出3个主题及8个亚主题:健康相关问题导致刻意回避社交(疾病本身的限制和社交参与困难,对健康问题的担忧,治疗不良反应导致外貌和形象的改变);遭受社交隔离与歧视(社交活动中感到被孤立,遭受他人的避讳和回避行为,公众对肺癌的误解和偏见);希望得到多方支持(期盼家庭、朋友和医护人员的支持,期盼专业心理机构和病友的支持).结论 肺癌化疗患者普遍经历社会隔离的不良体验,医护人员应重视肺癌化疗患者社会隔离的评估和干预,强化肺癌化疗患者的多维度社会支持,以期改善其社会隔离问题,进而提升其社会适应水平.
Abstract
Objective To explore the real experience of social isolation of lung cancer chemotherapy patients and provide references for supportive schemes.Methods From September to November 2023,fifteen lung cancer patients undergoing chemotherapy and hospitalized in Henan provincial people's hospital were selected and conducted a semi-structured interview using the qualitative research method.Then,content analysis is used to summarize and extract themes.Results Three thems and eight sub-thems are summarized:deliberate social avoidance due to health-related issues(limitations of the disease itself and difficulties in social participation,concerns about health problems,as well as changes in appearance and image due to side effects),suffering from social isolation and discrimination(feeling isolated in social activities,being subjected to taboo and avoidance behaviors by others,as well as public misunderstanding and prejudice about lung cancer),expect social support(from family,friends and medical team,as well as professional psychological institutions and fellow patients).Conclusion Lung cancer patients undergoing chemotherapy,generally experience bad experience of social isolation.Therefore,medical staff should pay attention to the evaluation and intervention of their social isolation,strengthen the multi-dimensional social support,so as to relieve their social isolation and promote their social adaptation level.
