Psoriasis is a common chronic autoimmune disease,which is difficult to cure,affects the appearance,and has a serious impact on the quality of life and mental health of patients.In addition,psoriasis is often associated with a variety of comorbidities,such as arthritis,metabolic syndrome,cardiovascular disease,mental and psychological diseases,and about 57.9%of patients with psoriasis have at least one comorbidity.In view of the above characteristics,the long-term treatment and management of psoriasis and its comorbidities are particularly important.Chronic disease management is a systematic treatment model to maintain the health status of patients by managing the underlying causes and risk factors of diseases.This article illustrates the importance of a comprehensive chronic disease management program for psoriasis and its comorbidities.This paper introduced the construction of the standardized diagnosis and treatment center,the introduction of the concept of treat-to-target,and the application of telemedicine technology in the management of chronic psoriasis.At the same time,the further optimization of chronic disease management of psoriasis was discussed based on policy factors and health economics.
