肾嗜酸细胞瘤的CT表现与病理对照分析

摘要
目的 探讨肾嗜酸细胞瘤的CT表现,以提高对本病的认识.资料与方法 回顾性分析经手术病理证实的5例嗜酸细胞瘤患者的影像学资料.5例均行CT平扫及三期增强扫描,将CT表现与大体病理标本进行对照分析.结果 5例病灶均为单发,4例位于右肾,1例位于左肾.4例CT平扫呈等密度为主,1例呈稍高密度为主;2例见钙化,其中1例见钙化性囊肿;1例见出血.上述征象与大体标本所见大体一致.增强检查3例病变于皮质期呈"轮辐状"强化,实质期及排泄期其强化程度逐渐减低;1例于皮质期明显均匀强化,实质期及排泄期强化程度低于同侧肾皮质,并见完整假包膜;1例强化不均,三期强化程度均低于同侧肾皮质.3例见"星芒状"瘢痕,其范围及形状与大体标本切面趋于一致.结论 肾嗜酸细胞瘤影像学表现有一定的特征,有助于同各型肾癌鉴别.
Abstract
Purpose To explore the CT findings of renal oncocytoma and to further understand the pathology of this disease.Materials and Methods The imaging data of 5 cases with surgery and pathology confirmed renal oncocytoma were retrospectively analyzed.All patients underwent plain CT and triple-phase enhancement on multi-slice CT.All images were compared with gross pathology.Results All 5 patients had solitary lesion of which four were in the right kidney and one in the left kidney.On plain CT scans, 4 tumors were primarily isodense and 1 was hyperdense.Calcification was found in 2 cases of which one had calcified cyst.Only one case had hemorrhage.All these findings were consistent with the gross specimen.Three tumors showed spoke-wheel-like enhancement in the cortico-medullary phase, but in the parenchymal phase and excretory phase, the attenuation gradually decreased.There was intact pseudocapsule in 1 case and showed uniform enhancement in cortico-medullary phase,but the density decreased in parenchymal and excretory phases showing hypodensity compared to adjacent renal cortex.In another case the enhancement was heterogeneous and less than renal cortex in all three phases.Central stellate-shaped scar was observed in 3 cases, which was consistent with the gross specimen.Conclusions Most renal oncocytomas show certain imaging characteristics, which are helpful for the differential diagnosis of renal tumors.
