Optimal Energy in Spectral CT Imaging for Displaying Normal Anatomy of Adrenal Artery

摘要
目的 了解正常肾上腺动脉的解剖,有助于判断病变供血动脉,进一步明确来源及制订治疗方案,所以探讨能谱CT最佳单能量成像对正常肾上腺动脉解剖的显示能力有着重要的临床意义.资料与方法 选取60例腹部肿瘤且肾上腺正常患者,采用能谱扫描模式行动态增强扫描,生成40~140 keV单能量图像,以1 keV为间距进行重建得到101组不同水平单能量图像.研究共分A、B两组,A组为最佳单能量组[肾动脉与竖脊肌之间具有最佳对比噪声比(CNR)的单能量图像],B组为对照组(系统默认选择的70 keV单能量图像,相当于140 kVp混合能量质量控制模式的平均能量),由2名影像科副主任医师采用双盲法以5分法对两组动脉期重建血管容积再现(VR)及薄层最大密度投影(MIP)进行主观评分,对两组图像质量进行比较,并记录肾上腺上、中及下动脉的显示率,评价两组对血管的显示能力.结果 (51±3)keV单能量图像具有最佳CNR,A组主观评分高于B组,差异有统计学意义(P<0.05);A组及B组对肾上腺上、中、下动脉显示率分别为61.67%、55.00%、88.33%和51.67%、48.33%、70.00%,差异有统计学意义(P<0.05),且A组中肾上腺下动脉显示率均高于肾上腺上、中动脉(P<0.05).结论 能谱CT最佳单能量技术提高了肾上腺动脉成像的图像质量,能更好地显示正常肾上腺各分支血管.
Abstract
Purpose Understanding of the anatomy of normal adrenal artery could help to determine the feeding artery of the lesion,thus would help make treatment options.It is important to evaluate the monochromatic dual-energy spectral CT imaging in displaying the anatomy of normal adrenal artery.Materials and Methods Sixty patients with abdominal tumors underwent contrast-enhanced spectral CT scan.All the patients showed normal adrenal glands.101 sets of monochromatic images with photon energies of 40-140 keV were acquired which were reconstructed with 1 keV interval.All the images were divided into two groups.Group A used the optimal energy level and group B used 70 keV.Image quality and display rates were compared between the two groups.A 5-score system was used to evaluate the maximum intensity projection and volume rendering reconstruction images by two observers blindly.Display rates of the adrenal arteries (superior,middle and inferior suprarenal artery) were recorded.Results Images of monochromatic energy with (51 ± 3) keV showed best CNR.The score of group A was higher than that of group B (P<0.05).The display rates of superior,middle and inferior adrenal artery between group A and B were 61.67%,55.00%,88.33% and 51.67%,48.33%,70.00% (P<0.05).The display rates of inferior artery were higher than those of the middle and superior suprarenal artery in group A (P<0.05).Conclusion The optimal energy of spectral CT imaging may improve the image quality of displaying adrenal artery.
