Objective:To investigate the curative effect and prognosis of intensity-modulated radiation therapy(IMRT)combined with transarterial chemoembolization(TACE)in treating advanced hepatocellular carcinoma(HCC).Methods:A retrospective analysis was conducted on 114 patients with advanced HCC who admitted to Anhui provincial cancer hospital from December 2015 to December 2021.All patients received IMRT combined with TACE.Univariate and multivariate COX survival analysis were performed to determine the independently predictive factors that affected survival period of patients.Results:The total effective rate(complete remission + partial remission)after IMRT combined with TACE was 60.5%(69/114)for all patients.Multivariate analysis revealed that larger tumor diameter,high score of Eastern Cooperative Oncology Group(ECOG),Ⅲ-Ⅳ portal vein tumor thrombosis(PVTT),hepatitis treatment status and elevated white blood cell count were independently predictive factors of affecting survival period of patients.Conclusion:The Ⅲ/Ⅳ PVTT of tumor and larger tumor diameter have significant influences on survival of patients,and other important factors that affect the survival of patients include physical condition of patients,hepatitis treatment and systemic inflammation.IMRT combined with TACE has better curative effect for advanced HCC,but the influence factors of prognostic survival should be fully considered.
