摘要

目的:探讨18F-脱氧葡萄糖(18F-FDG)正电子发射断层扫描-计算机断层扫描(PET/CT)摄取值及血清载脂蛋白A1(Apo A1)、谷胱甘肽(GSH)用于不同运动障碍亚型帕金森病的预测价值.方法:选取2020年12月至2022年12月在临沂市人民医院就诊的84例帕金森病患者,根据国际运动障碍协会统一帕金森病评定量表(MDS-UPDRS)将患者分为震颤为主型(TD)组(50例)和姿势不稳或步态障碍型(PIGD)组(34例).比较不同运动障碍亚型帕金森病患者18F-FDG PET/CT的18F-FDG摄取值、血清Apo A1及GSH水平,并通过受试者工作特征(ROC)曲线分析上述3项指标联合预测不同运动障碍亚型帕金森病的价值;验证18F-FDG摄取值、血清Apo A1及GSH联合检测对不同运动障碍亚型帕金森病患者预测结果与患者实际亚型的一致性.结果:TD组丘脑的18F-FDG摄取值显著低于PIGD组;TD组血清Apo A1水平显著高于PIGD组、TD组血清GSH水平显著高于PIGD组,差异有统计学意义(t=3.669、2.758、2.824,P<0.05);ROC曲线下面积(AUC)分析显示,丘脑18F-FDG摄取值、Apo A1及GSH联合预测不同运动障碍亚型帕金森病的AUC为0.858;18F-FDG摄取值、血清Apo A1及GSH联合对不同运动障碍亚型帕金森病患者预测结果与患者实际亚型的一致性较高(Kappa=0.804).结论:18F-FDG PET/CT摄取值、血清Apo A1及GSH联合应用对不同运动障碍亚型帕金森病有较好的的预测价值.

Abstract

Objective:To explore the predictive value of 18F-deoxyglucose(18F-FDG)positron emission computed tomography/electronic computed tomography(PET/CT)uptake values,serum apolipoprotein A1(ApoA1)and glutathione(GSH)for different subtypes of motor disorders of Parkinson's disease(PD).Methods:A total of 84 PD patients who admitted to Linyi People's Hospital from December 2020 to December 2022 were divided into tremor-predominant disorder(TD)group(50 cases)and postural instability or gait disorders(PIGD)group(34 cases)according to the International Movement Disorder Society Unified Parkinson's Disease Rating Scale(MDS-UPDRS).The 18F-FDG uptake values,serum Apo A1 and GSH levels in PD patients with different subtypes of motor disorders were compared,and the receiver operating characteristic(ROC)was used to analyze the value of the combined prediction of above 3 indicators for different subtypes of PD motor disorders.And then,the consistency between the combined detection of above 3 indicators and the actual subtype of patients was verified.Results:The 18F-FDG uptake values of thalamencephalon of TD group was significantly lower than that of PIGD group,and serum Apo A1 level of TD group was significantly higher than that of PIGD group,and serum GSH of TD group was significantly higher than that of PIGD group(t=3.669,2.758,2.824,P<0.05).ROC analysis showed that the area under curve(AUC)value of combined prediction of 18F-FDG uptake values of thalamencephalon,Apo A1 and GSH was 0.858 for different subtypes of PD motor disorders.The consistency between the results of combined prediction of 18F-FDG uptake values,serum Apo A1 and GSH and actual subtypes of patients was higher for PD patients with different subtypes of motor disorders(Kappa=0.804).Conclusion:The combined application of 18F-FDGPET/CT uptake values,serum Apo A1 and GSH has favorable predictive value for different subtypes of PD motor disorder.
