摘要
目的:探讨308 nm准分子激光治疗仪联合卤米松乳膏对白癜风的治疗效果.方法:选取2022年1月至2023年1月火箭军特色医学中心皮肤科收治的100例白癜风患者,采用随机数表法将其分为对照组和观察组,每组50例.对照组采用卤米松乳膏进行治疗,观察组应用308 nm准分子激光治疗仪联合卤米松乳膏进行治疗.对比两组T细胞亚群CD3+、CD4+和CD4+/CD8+比值水平,炎性细胞因子白细胞介素-10(IL-10)、IL-17、IL-23,以及血清单核细胞趋化蛋白-1(MCP-1)、可溶性细胞间黏附分子-1(sICAM-1)水平,比较两组治疗有效率及不良反应发生率.结果:观察组患者治疗有效率为96.00%(48/50),显著高于对照组的82.00%(41/50),差异有统计学意义(x2=5.005,P＜0.05);观察组治疗后CD3+、CD4+、CD4+/CD8+比值水平升高,CD8+水平降低,两组治疗后T细胞亚群指标比较,差异有统计学意义(t=6.745、2.406、3.969、3.998,P＜0.05);观察组治疗后IL-10水平高于对照组,IL-17、IL-23、MCP-1及sICAM-1水平低于对照组,两组比较差异有统计学意义(t=11.607、5.892、9.140、9.920、14.682,P＜0.05),观察组不良反应发生率为14.00%(7/50),对照组不良反应发生率为10.00%(5/50),两组比较差异无统计学意义(P＞0.05).结论:308 nm准分子激光治疗仪联合卤米松乳膏对于白癜风的治疗具有良好的效果,对患者T细胞亚群水平与炎性细胞因子及MCP-1、sICAM-1水平有改善效果,未增加不良反应,具有一定的临床应用价值.
Abstract
Objective:To explore the curative effect of 308nm excimer laser combined with halometasone cream on vitiligo.Methods:A total of 100 patients with vitiligo who admitted to PLA Rocket Force Characteristic Medical Center from January 2022 to January 2023 were selected.They were randomly divided into control group(50 cases)which underwent treatment by halometasone cream,and observation group(50 cases)which underwent treatment by 308nm excimer laser combined with halometasone cream.The levels of T cell subpopulations(CD3+,CD4+,CD8+),inflammatory cytokines[interleukin-10(IL-10),interleukin-17(IL-17),interleukin-23(IL-23)],serum monocyte chemotactic protein-1(MCP-1)and soluble intercellular adhesion molecules-1(sICAM-1)between two groups were compared,and the effective rate of treatment and the incidence of adverse reaction between two groups were also compared.Results:The effective rate of treatment of the observation group was 96.00%(48/50),which was significantly higher than that[82.00%(41/50)]of control group,and the difference was significant(x2=5.005,P＜0.05).After treatment,the CD3+,CD4+and CD4+/CD8+levels of the observation group increased,while the CD8+level decreased,and the difference of T cell subpopulations between two groups were significant after treatment(t=6.745,2.406,3.969,3.998,P＜0.05),respectively.After treatment,the IL-10 level of observation group was higher than that of control group,while the IL-17,IL-23,MCP-1 and sICAM-1 levels of observation group were significantly lower than those of control group(t=11.607,5.892,9.140/9.920,14.682,P＜0.05),respectively.The incidence of adverse reaction of observation group and control group were respectively 14.00%(7/50)and 10.00%(5/50),and the difference of that between the two groups was no significant(P＞0.05).Conclusion:308nm excimer laser combined with halometasone cream has a favorable effect in treating vitiligo,which can improve the levels of T cell subpopulations,inflammatory cytokines,MCP-1 and sICAM-1.It does not increase adverse reactions.Therefore,it has a certain clinical value.
