Evaluation of Potentially Inappropriate Medication in Elderly Inpatients with Diabetes Mellitus
Objective To provide a basis for rational drug use in elderly inpatients with diabetes mellitus(DM).Methods The electronic medical records of the elderly inpatients(aged≥65 years)with DM admitted to the Department of Endocrinology of the Shandong Provincial Hospital Affiliated to Shandong First Medical University from January 1 to December 31,2021 were collected.The American Geriatrics Society 2019 Updated AGS Beers Criteria® for Potentially Inappropriate Medication Use in Older Adults(hereinafter referred to as the Beers Criteria)and the Criteria of Potentially Inappropriate Medications for Older Adults in China(2017 Edition,hereinafter referred to as the Chinese Criteria)were used to evaluate the potentially inappropriate medication(PIM)of patients,and the binary Logistic regression analysis was used to evaluate the risk factors related to PIM.Results A total of 563 patients were included,including 260 males and 303 females,with an average age of(71.66±5.99)years,an average hospital stay of(9.18±3.20)d,involving(7.82±2.73)varieties of diseases.A total of 254 patients(45.12%,462 cases)and 229 patients(40.67%,359 cases)were involved in PIM based on the Beers Criteria and the Chinese Criteria respectively.The results of PIM evaluation by the two criteria was similar(P = 0.061).Alprazolam,insulin(sliding scale),antipsychotic drug(olanzapine),doxazosin and indomethacin were both evaluated as the drugs related to PIM by the two criteria,in which the cases of PIM involving alprazolam were both in the top three based on the two criteria.The binary Logistic regression analysis showed that the number of medications and hospital stay were both significantly correlated with PIM in elderly inpatients with DM based on the two criteria(P<0.05).Conclusion The incidence of PIM in elderly inpatients with DM is high,we should evaluate it according to the Beers Criteria and the Chinese Criteria to improve the effectiveness and safety of drug use,reduce the drug-related toxic and side effects,and ensure the rational drug use in clinical practice.
