Literature Analysis of 20 Cases of Fournier's Gangrene Induced by Sodium-Glucose Cotransporter 2 Inhibitor
Objective To investigate the characteristics of Fournier's gangrene(FG)induced by sodium-glucose cotransporter 2 inhibitor(SGLT-2i),and to provide a reference for safe medication in the clinic.Methods The studies related to FG induced by SGLT-2i in the PubMed,Embase,CNKI WanFang,and VIP databases were searched from the inception of each database to June 2023,and relevant data was statistically analyzed.Results Twenty studies were included,including twenty patients.Among them,there were fourteen males(70.00%)and six females(30.00%),with an average age of(56.0±11.5)years old.Twelve cases(60.00%)were described as obese,of which five cases were extremely severe obesity(with a body mass index of≥40 kg/m2).The median time of FG occurrence was 425 d,and the average value of glycated hemoglobin(HbA1C)at the time of FG occurrence was 9.2%.Eight patients' FG was likely to be induced by the use of SGLT-2i,and twelve patients' FG may be induced by the use of SGLT-2i.All patients recovered well after discontinuation of medication,timely debridement and drainage,and treatment with antibiotics.Conclusion During the clinical use of SGLT-2i,more attention should be paid to identifying its risk factors for FG.Once there is suspected swelling,pain,or discomfort in the genital or perineal area,medical attention and active treatment are required.
