摘要
目的 总结节拍化学治疗(简称化疗)治疗老年肿瘤患者的临床研究进展.方法 采用计算机检索PubMed,Web of Science,The Cochrane Library及中国知网、维普数据库中自建库起至 2023 年 3 月 1 日有关节拍化疗治疗老年肿瘤患者的临床研究文献,从作用机制和临床应用两方面对临床研究进展进行综述.结果 节拍化疗通过抑制肿瘤血管生成、调节免疫应答、诱导肿瘤细胞休眠、减少肿瘤细胞耐药等途径,使非小细胞肺癌、前列腺癌、乳腺癌等老年肿瘤患者有明确生存获益.结论 节拍化疗治疗老年肿瘤患者的有效性和安全性较好,为后线甚至一线治疗提供了新方案,但有待更大样本的临床研究进行验证.
Abstract
Objective To summarize the clinical research progress on metronomic chemotherapy in the treatment of elderly patients with tumor.Methods Literature related to clinical research on metronomic chemotherapy in the treatment of elderly patients with tumor in the PubMed,Web of Science,The Cochrane Library,CNKI,and VIP databases from their inception to March 1,2023 was searched to summarize the research progress from the mechanism and clinical application.Results Metronomic chemotherapy has definite survival benefits for elderly patients with non-small cell lung cancer,prostate cancer,breast cancer or other cancers through multiple pathways,including inhibiting tumor angiogenesis,regulating immune response,inducing tumor cell dormancy,and reducing tumor cell resistance.Conclusion Metronomic chemotherapy is effective and safe in the treatment of elderly patients with tumor,which can provide a new approach for posterior or even first-line treatment,but it still needs to be validated through larger clinical studies.
