Service Model and Practice of Clinical Pharmacists Assisting Family Pharmacists in Primary Medical Institutions
Objective To investigate a new model for improving the service ability of family pharmacists in primary medical institutions,and to establish standardized and homogeneous home pharmaceutical care.Methods A"fixed contracted pharmacist+mobile clinical pharmacist"home pharmaceutical care team was established,and the home pharmaceutical care such as drug therapy management,home medication cabinets sorting,and rational medication popularization was carry out through the"routine service+characteristic service"model.Through practical teaching,case discussion,and science popularization sharing,clinical pharmacists in secondary and tertiary hospitals provided assistance and linkage to family pharmacists in primary medical institutions.Results From June 2021 to December 2022,135 drug therapy management-related problems were resolued,over 40 home medication cabinets were sorted,and the promotion of rational drug use covered more than 8 000 people.Conclusion Clinical pharmacists providing assistance and linkage to family pharmacists can better leverage the resource and talent advantages of the secondary and tertiary hospitals,comprehensively promote the improvement and efficiency of home pharmaceutical care,and enhance the service abilities of family pharmacists.
