Cause Analysis and Preventive Measures of Prescription Dispensing Errors in Western Pharmacy in a Hospital
Objective To improve the quality of prescription dispensing in the Western pharmacy of the hospital,and to ensure the patients' medication safety.Methods The clinical rational drug use system was used to randomly select 24 000 prescriptions from the Western pharmacy of the hospital from January to December 2022,the dispensing error prescriptions were statistically analyzed and classified.The causes of errors were found out,and the corresponding preventive measures were formulated.Results Among the 24 000 prescriptions,82 prescriptions(0.34% )had dispensing errors,the main types of which were inappropriate dosage form,specification or route of administration(13 prescriptions,15.85% ),inappropriate usage and dosage(13 prescriptions,15.85% ),inappropriate drug selection(13 prescriptions,15.85% ),incomplete diagnosis writing(10 prescriptions,12.20% ),extra-course medication(10 prescriptions,12.20% ),combined drugs with the same pharmacological effect(10 prescriptions,12.20% ),repeated medication(3 prescriptions,3.76% ),and other types(9 prescriptions,10.98% ).The main reasons for errors were similar drug packaging(40.24% ),multiple specifications of drugs(36.59% ),unclear compatibility of drugs by pharmacists(8.54% ),non-standard dispensing process(7.32% ),patients' factors(6.10% ),and other factors(1.22% ).Conclusion The occurrence of prescription dispensing errors is related to factors such as pharmacists,physicians,patients,drugs,and work environment.The hospital should improve rules and regulations,strengthen pre-job training,strengthen drug storage space maintenance,enhance the professional competence and sense of responsibility of pharmacists and related staff,effectively reduce dispensing errors,and ensure patients' medication safety.
