Investigation on Family Pharmacist's Role in Self-Behavior Management and Medication Adherence of Elderly Patients with Type 2 Diabetes Mellitus in a Community
Objective To analyze the main influencing factors of self-behavior management and medication adherence of elderly patients with type 2 diabetes mellitus(T2DM)from the perspective of family pharmacists,and to provide a reference for home pharmaceutical care and medication adherence management.Methods Elderly patients with T2DM who had established health records and signed up for medical treatment in the Chengshousi Street Community Health Service Center in the Fengtai District of Beijing were selected to carry out a questionnaire survey by face-to-face or telephone interviews.The Summary of Diabetes Self-Care Activities-6(SDSCA-6)and the Morisky Medication Adherence Scale(MMAS-8)were used to assess patients' self-behavior management and medication adherence.Results A total of 372 survey questionnaires were distributed,and 330 were effectively collected,with an effective recovery rate of 88.71% .Univariate analysis results showed that the more drugs a patient taken,the higher the frequency of irrational drug use(P<0.001).Education levels had a significant impact on regular blood glucose monitoring,and patients with high school education levels were more likely to monitor glucose regularly compared to those with middle school education levels or below(P=0.033).The course of T2DM had a significant impact on dietary habits,with longer disease duration associated with better dietary habits(P=0.048).Patients with a higher body mass index(BMI)engaged in fewer days of regular exercise per week(P=0.046).Patients with no history of allergies had better exercise habits(P=0.025).Conclusion Self-behavior management can improve the medication adherence in elderly patients with T2DM.Home pharmaceutical care provided by family pharmacists should focus on patients who take multiple drugs,and have lower educational levels,shorter disease duration,and higher BMI,in order to improve their blood sugar control effectiveness.
