Effect of Glucocorticoids Pulse Therapy on the Neutrophil and White Blood Cell Counts in Patients with Chronic Kidney Disease
Objective To investigate the effect of glucocorticoids pulse therapy(GPT)on neutrophil and white blood cell counts in patients with chronic kidney disease. Methods A total of 76 patients with chronic kidney disease who underwent GPT in the nephrology department of the hospital from January 2018 to December 2022 were selected and divided into the infected group(25 cases)and the uninfected group(51 cases)based on whether they had infection. The patients in the uninfected group were divided into the non-glucocorticoid group(35 cases)and the glucocorticoid group(16 cases)based on whether glucocorticoids were used before GPT.According to the age of the patients, they were divided into the 10-19 years old group(6 cases),20-29 years old group(13 cases),and 40-49 years old group(11 cases). With the neutrophil and white blood cell counts in the last blood routine examination before GPT as the baseline level, the absolute values of neutrophil and white blood cell counts during and after GPT in patients were recorded. Results On the 1st,2nd,and 3rd day of GPT,the neutrophil and white blood cell counts of patients in the uninfected group significantly increased(P<0.05),while there was no significant difference in the infected group(P>0.05).On the 1st day after GPT,the neutrophil and white blood cell counts in the uninfected group and the infected groups decreased to the baseline level(P<0.05).There was no significant difference in the neutrophil and white blood cell counts between the glucocorticoid group and the non-glucocorticoid group on the 3rd day of GPT(P>0.05).The neutrophil and white blood cell counts in the 10-19 year old group on the 3rd day of GPT were significantly higher than those in the 20-29 year old group and the 40-49 year old group(P<0.05).Conclusion GPT can significantly increase neutrophil and white blood cell counts in uninfected patients, and the magnitude of the increase correlated with age but not with pre-GPT glucocorticoid usage. On the 1st day after GPT,the patients' neutrophil and white blood cell counts significantly decreased to the baseline level before GPT.
