摘要
目的:传承古人药嘱思想,构建现代规范化中药药嘱临床药学服务.方法:系统梳理中国古代医药典籍中中药药嘱相关内容,重点分析《伤寒杂病论》中的中药药嘱学术思想,传承古代药嘱的人用经验;结合现代发药交代技术要求,分析中药药嘱的核心组成要素;应用信息技术,构建与应用"中药汤剂药嘱服务系统",形成"中药药嘱服务单",促进中药临床药学服务.结果:古代医药典籍中蕴含的药嘱思想包括煎煮溶媒、服药温度、服药次数、服药禁忌和药后护理等5项内容,对中药临床治疗效果具有重要影响.现代药嘱的核心是对患者提供基础服药指导内容和个性化服药指导."中药汤剂药嘱服务系统"在首都医科大学附属北京中医医院投入使用后,2018年上半年累计服务患者13.4万人次,患者好评率100％.结论:中药药嘱临床药学服务模式的创新与应用,有助于提升临床药师用药指导质量,促进中药临床药学服务的规范化发展.
Abstract
Objective:Inherit the ancient thought of traditional Chinese medicine medication advice and construct modern stand-ardized clinical pharmacy service for traditional Chinese medicine medication advice.Methods:Analyze the academic ideas of tra-ditional Chinese medicine,medication advice in Treatise on Febrile and Miscellaneous Diseases,and inherit the human use experi-ence of ancient medicine prescriptions.Design the core components of traditional Chinese medicine medication advice based on the requirements of modern drug dispensing.Apply information technology to construct and apply the Medication Advice Service System for Traditional Chinese Medicine Decoction,and to form the"service sheet of Chinese medicine medication advice",so as to pro-mote the clinical pharmacy service of traditional Chinese medicine.Results:The medication advice service ideas contained in the ancient medical classics include five important factors which have impact on the clinical therapeutic effect of traditional Chinese med-icine,including decoction solvent,medication temperature,medication times,medication contraindications,and post-medication care.The core of modern medication advice is to provide patients with basic content of medication guidance and personalized medica-tion guidance.The Medication Advice Service System for Traditional Chinese Medicine Decoction has come into use at Beijing Hos-pital of Traditional Chinese Medicine.In the first half of 2018,it served a total of 134 000 patients,and the patient satisfaction rate was 100％.Conclusion:The innovation and application of clinical pharmacy service model of traditional Chinese medicine medica-tion advice is helpful to improve the quality of medication guidance of clinical pharmacists and promote the standardized development of clinical pharmacy service of traditional Chinese medicine.
