摘要
朱章志教授认为,消渴病病机“阴虚”实为“阳气之收藏功能减弱”,糖尿病肾病(DN)之病机为“太少并病”,早期以“太阴虚损”为主, “太阴虚损、清气下陷”为主要病机;晚期则以“少阴寒厥”为主,“少阴寒厥、阴水泛溢”为主要病机.论治DN宜立足“三阴之本”,重在扶三阴之阳,使阳气功能发挥自如,脏腑功能活动有序.故早期贵在“温运”,散太阴寒邪,健其运化,使清阳得升,方以理中汤为主;晚期宜速“温扶”,温通各经经气,方以四逆汤、吴茱萸汤为主.
Abstract
Professor ZHU Zhang-zhi considers that the essential pathogenesis of diabetes is not yin deficiency but subdues storing of yang qi.Taiyin disease involving shaoyin channel are pathogenesis of diabetic nephropathy.The pathogenesis of early stage is deficiency of taiyin and sinking clear refined substance.And the pathogenesis of advanced stage is shaoyin cold syndrome and overflowed water.The treatment is to establish at three yin channels.The fundamental treatment is warming yang of three yin channels.So yangqi would be storing,and the functions of the zang-fu organs would be freely.The treatment is warmed to govern the transportation and transformation,so that lucid yang is getting to ascend in early stage.It mainly uses Lizhong Decoction.And at the advanced stage it should be warmed and reinforced to dispel the cold.It mainly uses Sini Decoction and Wuzhuyu Decoction.
