摘要
总结顾氏外科诊治疮疡的学术思想与临床经验.疮疡治疗,多外治与内治结合,在外治直达病所的情况下,必须立足整体,通过内治法整体调节以改善局部而取得佳效;引温病学说,根据温病卫气营血的传变规律,先安未受邪之地,早用凉血散血药物,截断扭转病势,擅用通腑、养阴之法;治疗疮疡总以顾护脾胃为本,主张甘寒清热为宜,慎用大寒大苦之品,用药处方时时健脾和胃,倡导药疗与食疗结合,并重视饮食宜忌,主张辨证忌口;提出热毒血瘀为疮疡基本病机,清热活血为治疗大法,擅用藤类、络类、虫类等和营通络消肿止痛之品,临证常伍用通里攻下法,使毒从下泄,并认为因虚感邪,既病伤正,主张适时应用生黄芪、皂角刺等扶正托毒,顾护正气,透表达邪外出,并贯穿治疗始终.
Abstract
This paper summarized GU's academic thought and clinical experience in treating sores.Sores were mostly treated with combined internal treatment and the external treatment.Under the condition of drugs focus on one position with external treatment,it should improve local based on the whole regulation by using internal treatment.Using drugs for cooling blood and dissipating stasis as early as possible to cut off and reverse the tendency of disease and good at using the treatment of facilitating bowel movement and nourishing yin according to the change regularity of warm disease in weifen,qifen,yingfen,xuefen,that was ‘protect the place untouched by evil firstly’.Sores treatment was always based on the protection of spleen and stomach,and advocated using kennedy and cold drugs to clear away heat,and using drugs of too bitter and cold carefully.Always paid attention to strengthening the spleen and stomach when prescribing,advocated medication combined with diet,attached great importance to avoid eating appropriately.GU Shi pointed out that the basic pathogenesis of sores was heat-toxin and blood stasis,so the treatment of sores was cleating heat and promoting blood circulation,and GU Shi was good at using drugs for detumescence and relieving pain,harmonizing yingfen and removing obstruction in collaterals,such as ivy,retinervus,insects and so on.It always combined with purgation method in clinical,which made the toxin discharged from stool.GU Shi also considered that if attacked by evil because of weak,it was suitable to use drugs for strengthening vital qi to eliminate pathogenic factor,protecting of vital qi,eliminating pathogens from exterior,such as milkvetch root,Chinese honeylocust spine and so on.
