基于菌群-肠-脑轴探讨"清浊相干"理论在逍遥散干预抑郁症的内涵

摘要
抑郁症是一种常见的精神类疾病,其发病率正逐年增长.现有研究显示,抑郁症的发病与肠道菌群紊乱密切相关."清浊相干"理论源于《黄帝内经》,是中医经典中对脾胃升清降浊的功能失常,清阳不升,浊气不降,浊邪宿积的一种概括."清浊相干"导致脑失所养,浊邪上犯清窍是肠道菌群紊乱引发神志病的重要病机.逍遥散是疏肝健脾的代表方剂,现代研究表明逍遥散不但可以作用于胃肠道治疗功能性消化不良、慢性胃炎、肠易激综合征等疾病,还可以调节肠道菌群及其代谢物以发挥抗抑郁作用.从肠道菌群紊乱角度,运用"清浊相干"理论来阐明逍遥散抗抑郁的理论基础,对探究抑郁症的发病机制及拓宽逍遥散的临床应用具有巨大的理论和临床意义.
Abstract
Depression is a common psychiatric disorder with its increasing prevalence year by year.Studies have shown that the incidence of depression is closely related to the disturbance of gut microbiota.The theory of'abnormal ascent-descent between clear yang and turbid yin'originates from the Huangdi Neijing,which is a generalization of the classical Chinese medicine on the malfunction of the spleen and stomach in raising the clear yang and lowering the turbid yin,resulting in the accumulation of turbid pathogenic factor.The'abnormal ascent-descent between clear yang and turbid yin'causes the brain to lose its nourishment and the turbid pathogen offending the clear orifice,which is the key to the disorder of gut microbiota that causes neurosis.Modern research has shown that it can not only act on the gastrointestinal tract to treat functional dyspepsia,chronic gastritis,irritable bowel syndrome and other diseases,but also regulate the gut microbiota and its metabolites to exert antidepressant effects.It is of great theoretical and clinical significance to investigate the pathogenesis of depression and to broaden the clinical application of Xiaoyao Powder and to elucidate the theoretical basis of the antidepressant effect of Xiaoyao Powder by applying the'abnormal ascent-descent between clear yang and turbid yin'theory from the perspective of gut microbiota disorders.
