中医内科学疾病定义与诊断标准问题探析

摘要
定义是厘清概念,促进交流的重要工具,中西医内科学多采用逻辑学理论定义中"属加种差"方法对疾病进行定义.中医内科学疾病定义模式中"属"为疾病,"种差"多为主症.这种定义模式有不能反映疾病本质、导致中医辨病论治实质缺位、模糊症状与疾病界限、导致定义可能过宽或过窄、造成临床诊断的不确定和不完备等缺陷.诊断标准是疾病判断的另一重要方面,中医内科学由于采用了以主症为"种差"的定义模式,造成了诊断标准模糊甚至缺如.中医内科学应调整疾病定义模式,研制诊断标准以适应当今医学发展需要.
Abstract
Definition is an important tool to clarify the concept and promote communication.Traditional Chinese and Western medicine internal science mostly uses the method of'genus plus species difference'in the definition of logic theory to define diseases.In the model of disease definition in traditional Chinese medicine(TCM)internal medicine,'genus'is a disease,and'species difference'is mainly a disease.This definition model has some defects,such as not reflecting the nature of the disease,leading to the absence of the essence of TCM treatment based on disease differentiation,blurring the boundary between symptoms and diseases,leading to the possibility of too wide and too narrow definitions,and causing uncertainty and incompleteness of clinical diagnosis.Diagnostic criteria are another important aspect of disease judgment.Because of the adoption of the definition model of'species difference'based on the main symptoms in TCM internal medicine,the diagnostic criteria are vague or even absent.TCM should adjust the disease definition model and develop diagnostic criteria to meet the needs of today's medical development.
