从"气津敷布"视角构建动脉粥样硬化中医辨治体系

文章全新构建了动脉粥样硬化(AS)中医辨证体系.AS病在脾胃,斑块为形,沉积于脉,属于中医"脉积"范畴.认为本病乃脾不散精,浊邪入络,脉积闭阻而发,以正虚、痰浊、络瘀为基本病机,其中脾失健运是斑块萌发的病理基础,陈气血浊是浊邪积滞的关键因素,浊邪入络是动态演变的中间环节,痰瘀积脉是斑块结聚的终末阶段.运用固本清源法进行治疗:一则健脾助运,恢复脾胃升降;二则痰瘀同治,抑制斑块发展;三则散结消积,逆转斑块结构.AS乃复合病机,宜辨五脏以多脏兼顾、辨标本以扶正祛邪、辨兼夹以多法并举、辨病位以精准施治,以期为AS中医辨治及组方提供借鉴和参考.
This article establishes a new traditional Chniese medicine(TCM)syndrome differentiation system for atherosclerosis(AS).AS occurs in the spleen and stomach,formation of plaque,deposited in the pulse,belongs to the Chinese medicine'pulse accumulation'category.It is considered that this disease is caused by the spleen not dissipating essence,turbidness of phlegm and blockage of veins.The basic pathogenesis is deficiency of vital qi,phlegm turbidness and collaterals stasis.Among them,spleen disturbance is the pathological basis of plaque germination,chen qi and blood turbidness is the key factor of lipid turbidness,turbidness into collaterals is the middle link of dynamic evolution,and phlegm stasis is the final stage of plaque accumulation.The treatment with the method of strengthening the root and clearing the source:first,strengthening the spleen to help transport,restoring the spleen and stomach;seconds,the treatment of phlegm and blood stasis combined,inhibit plaque development;thirdly is to dissipate the accumulation and reverse the plaque structure.AS is a complex pathogenesis,it is appropriate to distinguish the five Zang viscera to take into account the multiple viscera,distinguish the combination of multiple methods at the same time,distinguish the specimen to foster and dispel the evil,distinguish the outcome to prevent the disease from changing,in order to provide reference and reference for the differentiation and treatment of AS in TCM.
