宋兴运用风药及芳化药物治疗过敏性紫癜经验
SONG Xing's experience in treating Henoch-Schonlein purpura with wind medicine and aromatizing medicine

摘要
当前中医对过敏性紫癜的诊疗研究多围绕热性病机展开,宋兴教授认为湿邪郁闭类证是本病不可忽视的一类情况,指出临床中存在对此证的过用寒凉、闭塞阳气、郁遏湿邪等误治问题.其强调应具体问题具体分析,反对妄执某言;注重从气机运动及气化能力失常论治百病,强调应从湿邪郁闭玄府、妨碍津气升降出入导致皮下出血分析,指出基本病机为湿郁络伤;治疗上重视风药与芳化药物的透化湿邪、疏达玄府能力.针对湿邪郁闭兼气虚、兼痰浊或湿浊,寒化导致寒湿郁闭,热化导致湿热郁闭等情况,提出了疏透芳化佐益气、佐通利三焦,寒湿型重温化开宣,湿热型重芳香疏透等治疗思路.
Abstract
At present,the diagnosis and treatment of Henoch-Schonlein purpura in traditional Chinese medicine mostly focus on heat etiology.Professor SONG Xing believes the symdrome of dampness stagnation is an important condition of this disease which cannot be ignored.And there are lots of mistreatment of this condition such as overuse of cold,occlusion of yang qi and depression of dampness.He emphasizes syndrome differentiation and considers doctors should analyze and treat case-by-case in clinical practice,and opposes arbitrary assertions.He pays attention to the the treatment of all diseases based on the abnormality of qi movement and qi-transformation ability,and emphasizes the stagnation of dampness would close the Xuanfu and hinder the ascending,descending,entering and exiting of qi and fluid,which leads to subcutaneous hemorrhage.He holds the dampness stagnation and collateral injury is the basic pathogenesis and prioritizes the ability of wind medicine and aromatizing medicine to diffuse dampness and dredge the Xuanfu.In view of the situation of dampness syndrome combined with qi deficiency,phlegm turbidity or dampness turbidity,cold transformation leading to cold-damp stagnation,heat transformation leading to damp-heat stagnation,he proposed to dredge and aromatize to supplement qi,and to relieve sanjiao.The cold-dampness type is warmed yang qi and circulated qi,and the damp-heat type is scented and dredged.
