清心莲子饮加减治疗放射性膀胱炎经验探析

摘要
放射性膀胱炎是盆腔肿瘤放射治疗后常见的并发症.郑玉玲教授认为放射线是治疗肿瘤的常用方法,但其不良反应较大,属火热之邪直中机体,耗气伤阴,损及脏腑.盆腔放射治疗会影响到膀胱,波及肾脏,肾阴虚无以制约心火,虚火下移小肠,转于膀胱.阴虚及阳,无以蒸腾气化水液,湿浊内阻流注下焦,呈现气阴两虚而膀胱湿热的证候.其直接病位在膀胱,间接病位在心肾.临证以益气养阴为主,清心利湿为辅.郑教授选用清心莲子饮为基础方,根据临床症状加减化裁,同时注重疏导患者心理,加强对饮食调护方面的辅导,多获良效.
Abstract
Radioactive cystitis is a common complication of pelvic tumor after radiotherapy.Professor ZHENG Yuling thinks thatradiation is a common method to treat tumors,but its side effects are great.It is the fire-heat of the six pathogenic factors which consumes so large amount of qi and yin of the body that injure Zang and Fu.Pelvic radiation can affect the bladder and spread to the kidneys.The kidney yin is too deficient to control the heart fire.Fire of deficiency type shifts down to the small intestine,and then moves to the bladder.Deficiency yin affecting yang,lack of steaming and transforming of kidney led to damp flowing downward to lower-jiao.Therefore,it presents the appearance of deficiency of qi and yin and damp-heat of bladder symptom.It directly sited in the bladder,indirectly sited in heart and kidney.In clinical,the treatment mainly focus on benefitting qi and nourishing yin,clearing heart fire and promoting diuresis,then select Qingxin Lianzi Decoction as the basis prescription by professor ZHENG and administer drugs based on the combination of syndromes,pay attention to dredging patients'psychology and strengthening the guidance of diet and nursing,which achieved good results.
