目的:探讨乳管扩张器配合燕京无痛排乳法治疗乳汁淤积症的临床疗效.方法:选取2021年1月一2022年1月于首都医科大学附属北京中医医院乳腺科门诊的118例乳汁淤积症患者,按随机数表法分为试验组60例,对照组58例,试验组予乳管扩张器配合燕京无痛排乳法,对照组予中药外敷配合燕京无痛排乳法,比较两组临床视觉模拟评分法(VAS)评分、乳房肿块面积、通乳情况、体温、白细胞(WBC)水平、C反应蛋白(CRP)水平以及临床疗效.结果:治疗后,两组VAS评分、肿块面积、通乳情况、体温、WBC、CRP较治疗前均显著降低(P＜0.01),且试验组VAS评分、通乳情况、体温、WBC、CRP指标显著低于对照组(P＜0.01,P＜0.05).试验组临床总有效率[93.33％(56/60)]显著优于对照组[84.48％(49/58)](P＜0.05).结论:乳管扩张器配合燕京无痛排乳法可以有效治疗乳管堵塞,缓解疼痛不适,消散肿块,治疗乳汁淤积症.
Clinical effect of mammary duct dilator combined with Yanjing painless milk drainage method in the treatment of milk stasis
Objective:To observe the clinical effect of mammary duct dilator combined with Yanjing painless milk drainage method in the treatment of milk stasis.Methods:A total of 118 patients diagnosed with milk stasis admitted to Beijing Hospital of Traditional Chinese Medicine,Capital Medical University from January 2021 to January 2022 were randomly divided into experimental group(60 cases)and control group(58 cases).The experimental group was treated with mammary duct dilator combined with Yanjing painless milk drainage method.The control group was treated with traditional Chinese medicine external application combined with Yanjing painless milk drainage method.The clinical visual analogue scale(VAS)pain score,mass area,breast penetration score,body temperature,white blood cell level(WBC),C-reactive protein level(CRP)and clinical effect were measured.Results:After treatment,the VAS score,mass area,breast permeability,body temperature and WBC,CRP level of the two groups were lower than those before treatment(P＜0.01),and the VAS score,breast penetration score,body temperature,WBC and CRP levels in the experimental group significantly decreased than those in the control group(P＜0.01,P＜0.05).The total efficacy of the experimental group[93.33％(56/60)]was better than that of the control group[84.48％(49/58)](P＜0.05).Conclusion:The mammary duct dilator combined with Yanjing painless milk drainage method could effectively treat breast tube blockage,relieve pain and discomfort,dissipate masses and treat milk stasis.
