国医大师林毅精气血同调辨治乳腺癌晚期恶液质经验

恶液质是乳腺癌终末期常见并发症,现代医学认为与炎症因子激活、神经干扰、蛋白质脂质分解等因素相关,预后恶劣.国医大师林毅教授认为,晚期乳腺癌恶液质病机在于水谷气尽、大肉尽脱,涓流日久、气随血耗,久病穷精、动摇根基,该病病位在脾肾.治疗上强调顺应天时,顺应病势,依据邪正盛衰状态及病变脏腑当令之时,择时攻补;注重应用血肉有情之品以补肾填精生髓的同时,强调醒胃运脾,以助运化血肉有情之品,避免痰湿积聚.
Abstract
Cachexia is a common complication of advanced breast cancer and associated with activation of inflammatory factors,neural interference,and protein-lipid breakdown,leading to a poor prognosis.Professor LIN Yi,a TCM master,believes that the pathogenesis of advanced breast cancer cachexia lies in the exhaustion of the cereal essence,causing depletion muscles of extremities,long-term leakage of qi and blood,and a weakened foundation of Jing due to prolonged illness.The disease primarily affects the spleen and kidneys.In terms of treatment,LIN emphasizes adapting to the natural course of the disease and its progression,based on the balance between pathogenic factors and the healthy state,and selecting the appropriate timing to eliminating the evil factors and restorating healthy energy.It is important to use animal medicine to nourish the kidneys and generate bone marrow,while revitalizing stomach and invigorating spleen to assist in the digestion and transformation of nutrient-rich substances,thereby avoiding the accumulation of phlegm and dampness.
