基于"滞-郁-虚"动态演变探讨慢性应激诱导抑郁的病机特点

摘要
文章回溯了滞、郁、虚的概念发展源流及演变,从字形、字义角度解释滞、郁、虚的内涵,对精神疾病中定义模糊的滞、郁病机进行深层涵义的挖掘和对比研究.滞代指气机不畅、病理产物蓄积、脏腑功能失调的病机特征;郁在囊括滞的基础上强调疾病层面的忧郁病症表现,滞为郁的内在病理基础,郁为滞的外在行为表象.虚为不足,为抑郁中后阶段的主要特征.在此基础上阐述慢性应激诱导抑郁的"滞-郁-虚"病机演变特点:滞是病理基础;郁是发病机转,是抑郁行为发生的标志;虚是结局指标,是疾病加重、病程延长的病机枢纽.针对"滞-郁-虚"不同阶段,治疗当采取行气、解郁、补虚等治则,并根据具体病机、病机兼杂、病机程度而随证加减治疗.
Abstract
This research traces back the development and evolution of the concepts of stagnation,depression and deficiency,explains the theoretical connotation of stagnation,depression and deficiency based on their definition,and makes a deep meaning comparison on the pathogenesis of stagnation and depression in mental diseases.Stagnation refers to the pathogenesis of qi stagnation,accumulation of pathological products and visceral dysfunction after stress.Depression emphasizes the manifestation of depression at the disease level on the basis of covering the pathogenesis,stagnation is the internal pathological basis of depression,and depression is the external behavior representation of stagnation.Deficiency is inadequate,which is the main feature of the middle and later stages of depression.On this basis,the characteristics of the pathogenesis evolution of'stagnation-depression-deficiency'of chronic stress-induced depression are expounded.Stagnation is the pathological basis.Depression is the pathogenesis and the sign of depressive behavior.Finally,deficiency is the outcome index and the hinge of pathogenesis for aggravating disease and prolonging the course of disease.In view of the different stages of the development of the pathogenesis of'stagnation-depression-deficiency',the treatment should adopt different principles such as promoting qi,relieving depression and tonifying deficiency,and add or subtract treatment according to the specific pathogenesis,the complexity of pathogenesis and the severity of pathogenesis.
