目的:评价胃瘫外敷方早期干预对消化系统恶性肿瘤术后胃瘫(PGS)不同风险人群发病率的影响.方法:纳入2020年10月1日至2021年1月31日在中国医学科学院肿瘤医院行消化系统恶性肿瘤手术且腹部局部辨证属寒证患者152例作为试验组,纳入2019年10月1日至2020年1月31日该院相同医生、相同病种、相同手术方案的消化系统恶性肿瘤手术且腹部局部辨证属寒证患者152例作为对照组,结合PGS风险预测模型,分别按患者患PGS风险划分为高危人群(模型评分＞51分)、低危人群(模型评分≤51分).对照组予单纯西医常规治疗,试验组在对照组西医常规治疗基础上加用胃瘫外敷方(术后第1天提前干预),治疗7 d后,进行2周内PGS发病率比较.结果:试验组整体人群及高危人群、低危人群PGS发病率均较对照组下降,整体人群中对照组、试验组PGS发病率分别为13.16％、3.95％,高危人群中对照组、试验组PGS发病率分别为28.00％、10.00％,低危人群中对照组、试验组PGS发病率分别为5.88％、0.98％,其中整体人群及高危人群PGS发病率下降差异有统计学意义(整体人群P=0.004,高危人群P=0.022).结论:胃瘫外敷方早期干预可降低PGS发病率,尤其对PGS高危人群疗效更显著.
Effects of early intervention of Weitan Waifu Patch on the incidence of gastroparesis after surgery of digestive system malignant tumor
Objective:To evaluate the effect of early intervention with Weitan Waifu Patch on the incidence of postoperative gastroparesis(PGS)in different risk groups.Methods:A total of 152 patients with digestive system tumor surgery and local abdominal discriminate cold syndrome from October 1,2020 to January 31,2021 in Cancer Hospital Chinese Academy of Medical Sciences were included as the treatment group,and 152 patients with digestive system tumor surgery and local abdominal discriminate cold syndrome from October 1,2019 to January 31,2020 in the same hospital with the same doctor,the same disease,the same surgical plan were included as the control group.Combined with the PGS risk prediction model,the patients were divided into high-risk population(model score＞51)and low-risk population(model score ≤51).The control group was treated with conventional western medicine,and the treatment group was treated with Weitan Waifu Patch(intervention on the first day after surgery)on the basis of the conventional western medicine treatment of the control group.After 7 d of treatment,the incidence of PGS in 2 weeks was compared.Results:The incidence of PGS in the overall population,high-risk population and low-risk population in the treatment group were lower than those in the control group.The incidence of PGS in the control group and the treatment group was 13.16％and 3.95％in the overall population,28.00％and 10.00％in the high-risk population,and 5.88％and 0.98％in the low-risk population.The incidence of PGS in the overall population and high-risk population decreased significantly(overall population P=0.004,high-risk population P=0.022).Conclusion:Early intervention with Weitan Waifu Patch can reduce the incidence of PGS,especially for high-risk population of PGS.
