基于"浊害清和"理论探讨糖尿病视网膜病变的病机与辨治

"浊害清和"是明代眼科大家傅仁宇提出的有关中医病机的理论思想,其对阐释糖尿病视网膜病变的发生、发展、转归及治疗具有重要的指导意义,但既往研究对这一理论的认识较少,文章旨在探讨"浊、清、和"的意义以及"浊害清和"的内涵,从微循环视角探讨"浊害清和"在糖尿病视网膜病变中的作用机制.同时,文章认为,浊邪内蕴、精气虚衰、清窍失濡是糖尿病视网膜病变的关键病机,湿热痰瘀毒为病理产物,脾肝肾是病后虚衰的主要脏腑,并从"浊害清和"逆向确立"化浊补虚"的基本治疗思路,以清热除湿、化痰活血、解毒祛浊、补脾调肝益肾为核心治法,以期为中西医结合治疗糖尿病视网膜病变提供借鉴.
Discussion on pathogenesis and treatment of diabetic retinopathy based on the theory of'turbidity invading the clear orifice and harmony'
The theory of'turbidity invading the clear orifice and harmony'is a theoretical thought about traditional Chinese medicine(TCM)pathogenesis put forward by FU Renyu,a ophthalmology master in Ming dynasty,which has important guiding significance for explaining the occurrence,development,prognosis and treatment of diabetic retinopathy,but previous studies have less understanding in this theory,this paper aims to explore the meaning of'turbidity,clear orifice and harmony',to explore the performance of'turbidity invading the clear orifice and harmony'in diabetic retinopathy from the perspective of microcirculation.It is believed that the key pathogenesis of diabetic retinopathy is the connotation of turbidity,the deficiency of vital essence and the nourishment's loss of the upper orifices,and damp heat,phlegm,stasis and poison are pathological products,spleen,liver and kidney are the main viscera of deficiency after disease,establish the basic treatment idea of'changing turbidity and supplementing deficiency'.The core treatment method is to clear heat and dehumidify,dissolve phlegm and promote blood circulation,detoxify and dispel turbidity,replenish spleen,regulate liver and kidney,in order to provide reference for the treatment of diabetic retinopathy with integrated traditional Chinese and Western medicine.
