基于孙络"溢奇邪,通营卫"理论论治急性放射性皮炎

摘要
孙络是人体经络末端的细小分支,而皮肤之孙络是孙络系统的重要组成部分,其与皮肤真皮层内毛细血管、毛细淋巴管和细胞间隙等"微小通道"极为相似."溢奇邪、通营卫"是《黄帝内经》对于孙络功能的经典阐释,这与现代医学皮肤损伤后炎症因子释放、免疫细胞浸润等过程密切相关.文章基于以上理论,认为急性放射性皮炎病位在于孙络,其发生发展过程中不同时期的病机可概括为超早期火毒直入孙络,中期火毒燔灼孙络、波及营卫,后期余毒未散、气血溃败.针对以上病机,总体治疗法则应以"溯病因、稳病势、溢奇邪、通营卫"为主,旨在清解火毒,调和营卫以减轻急性放射性皮炎的严重程度,缩短病程,加速愈合,从而达到整体的防治作用.
Abstract
Similar to the'micro-channels'including capillary vessels,lymphatic capillary and intercellular space in the dermis of skin,Sunluo existed in the skin is an important component of sunluo system,which refers to the small branch at the end of the human body's meridians.'Eliminate peculiar pathogen and circulate Yingwei',as a classical theory about the function of Sunluo in Huangdi Neijing,is closely related to the process of inflammatory factor release and immune cell infiltration based on modern medicine theory.According to the theory above,we believe that acute radiation-induced dermatitis(ARD)can be located in Sunluo,accompanied by a series of traditional Chinese medicine pathogenesis in different periods during its occurrence and development summarized as the virulent fire penetrates directly into Sunluo in the super early stage,heat flaring in Sunluo and affect Ying and Wei in the middle stage,as well as corruption in qi and blood with residual poison undissipated in the later stage.The overall principles of treatment should be focus on'tracing the cause,stable the condition,excrete peculiar pathogen and unobstruct Ying and Wei'aiming at clearing virulent fire,regulating Ying and Wei,in order to reduce the severity,shorten the course and accelerate the healing process with the hope of achieving an overall effect of prevention and treatment of ARD.
