中医症状判断标准、分类分级标准现状及发展路径思考

摘要
症状包括患者主观不适和客观体征,是中医辨病、辨证基础.中医历史悠久,典籍丰富,对如何辨识、分析症状有丰富论述.但相对现代医学而言,中医主要依据相关因素的有无对症状进行判断,症状的客观判断标准、分类分级标准过少,且几乎没有量化指标,一定程度上造成中医诊断随意性、不一致性;并导致比较患者间、同一患者患病前后的症状严重程度、疗效等出现困难.中医可以通过参考现代医学对应标准、充分挖掘古代文献和利用中医理论特点3个方面着手,发展出自身症状判断和分类分级标准.
Abstract
Symptoms include subjective discomfort of patients and objective signs found by doctors,which are the basis of traditional Chinese medicine(TCM)disease differentiation and syndrome differentiation.TCM has a long history and rich ancient books.It has a rich discussion on how to identify and analyze symptoms.However,compared with modern medicine,TCM mainly judges symptoms based on the presence or absence of symptom related factors.There are too few objective judgment standards,classification and grading standards for symptoms,and there are almost no quantitative indicators.To a certain extent,the diagnosis of TCM is arbitrary and inconsistent,resulting in difficulties in comparing the severity and efficacy of symptoms between patients and before and after the same patient.TCM can develop its own symptom judgment and classification standards by referring to the corresponding standards of modern medicine,fully excavating the ancient literature and making use of the theoretical characteristics of TCM.
