络病理论指导下特发性肺纤维化急性加重因机证治刍议

Discussion on the pathogeny pathogenesis syndrome differentiation and treatment of acute exacerbation of idiopathic pulmonary fibrosis under the guidance of collateral disease theory

特发性肺纤维化急性加重(AE-IPF)是特发性肺纤维化(IPF)患者死亡的重要原因,属呼吸系统难治性疾病.文章以络病理论为指导,以现有研究成果为基础,探讨AE-IPF的因、机、证、治.AE-IPF的络病病因为自然环境异常、社会心理应激、生活起居异常、代谢产物蓄积和金刃虫兽外伤.其络病病机为肺络失制;其络病证型为肺络损伤证、肺络邪滞证、肺络虚耗证;在治疗上当通络以祛邪,补络以扶制,运用通络法以祛除病邪的同时修复肺络功能、调动其自稳功能.文章为AE-IPF的实验研究和临床实践提供了理论基础.
The acute exacerbation of idiopathic pulmonary fibrosis(AE-IPF)is an important cause of death in patients with idiopathic pulmonary fibrosis(IPF),which is a refractory disease of the respiratory system.Guided by the collateral disease theory and based on the existing research achievements,this study explores and innovates the causes,mechanisms,syndromes and treatments of AE-IPF,that is,the collateral disease of AE-IPF is caused by abnormal natural environment,social and psychological stress,abnormal living habits,accumulation of metabolic products,and injury of golden bladed animals.The collateral disease pathogenesis is lack of control of lung collateral,and the collateral disease syndromes are the lung collateral injury syndrome,the lung collateral evil stagnation syndrome,and the lung collateral deficiency syndrome.In terms of treatment,the collateral disease should be unblocked to eliminate pathogens,reinforce collaterals to support the system,and use the method of dredging collaterals to remove diseases and pathogens while repairing the lung collateral function and mobilizing its self stabilizing function,providing a theoretical basis for the experimental research and clinical practice of AE-IPF.
