中药复方对不同证型慢性失眠患者舌象图像指标的影响

摘要
目的:探讨辨证运用中药复方对痰热扰心证、心肾不交证慢性失眠患者舌象图像指标的影响.方法:于2021年1月1日-12月31日上海市黄浦区香山中医医院收集慢性失眠患者31例,其中,痰热扰心证13例,心肾不交证18例.各服用中药复方8周,分别检测服药前及服药第2、4、6、8周的舌象图像,以及服药前、服药第4和8周匹兹堡睡眠质量(PSQI)量表并分析.结果:痰热扰心证慢性失眠患者服药后第2、8周,舌质、舌苔S值较服药前显著增高(P＜0.01),L值显著降低(P＜0.01);服药后第2、6、8周,舌苔CON、ENT、MEAN值显著降低(P＜0.05,P＜0.01);心肾不交证慢性失眠患者服药后第4、6、8周,舌质a值较服药前显著降低(P＜0.05,P＜0.01).整体变化呈线性趋势(P＜0.05).结论:辨证运用中药对不同证型慢性失眠舌象图像指标有影响.舌象客观指标应用于中药治疗失眠的疗效评价具有可行性.
Abstract
Objective:To determine the effects of traditional Chinese medicine(TCM)compound formulas under syndrome differentiation on the image indicators of tongue in chronic insomnia patients with the syndrome of phlegm-heat disturbing the heart or the syndrome of heart-kidney disharmony.Methods:Thirteen cases of phlegm-heat disturbance syndrome and eighteen cases of heart-kidney disharmony syndrome were among the thirty-one patients with chronic insomnia that were gathered from Xiangshan Hospital of Traditional Chinese Medicine,Huangpu District,Shanghai,between January 1 and December 31,2021.Eight weeks of TCM compound formulas were given to each patient.The tongue images before taking the medicine and the 2nd,4th,6th and 8th weeks of taking the medicine were tested and analyzed,as well as the PSQI scale before taking the drug,the 4th and 8th weeks of taking the drug.Results:The S values of tongue body and coating of patients with phlegm-heat disturbing heart syndrome were significantly higher on the second and eighth week after compound formulas treatment than it was before(P＜0.0l),while L value was significantly lower(P＜0.01);On the second,sixth,and eighth weeks of medication administration,there was a significant decrease in the CON,ENT,and MEAN values of tongue coating(P＜0.05,P＜0.01);The a values of tongue body of patients with heart-kidney disharmony syndrome were significantly lower on the fourth,sixth and eighth weeks of taking medication(P＜0.05,P＜0.01).A linear trend was evident in the overall change(P＜0.05).Conclusion:TCM under syndrome differentiation has positive effects on the tongue image indexes of chronic insomnia with different syndrome types.It is feasible to assess the effectiveness of TCM in treating insomnia by using the objective indicator of tongue image.
