基于"中虚血积"理论探析中晚期肝癌的辨治

摘要
西医综合治疗后的中晚期肝癌患者是中医的主要患者群体,病情进展快,治疗难度大.文章提出"中虚血积"的肝癌辨治思路,认为"中虚血积"是中晚期肝癌的突出特点."中虚血积"指中土脾胃亏虚后出现阳气乏源、情志失和、伏邪难出等病理变化,导致人体寒热错杂,气血紊乱,形成瘀血、水饮、毒邪等有形实邪,终致肝癌的产生与进展.治疗上,需察别阴阳,守中护土;活血破积,通络泄浊.以减轻患者痛苦,防止复发转移.重视并改变"中虚"病机,可能成为中医药在现代治疗肝癌的重要思路.
Abstract
Patients with advanced liver cancer,who have been treated by the comprehensive therapies of western medicine are the major group of traditional Chinese medicine,with rapid disease progression and difficult treatment.This paper puts forward the idea of differentiation and treatment of liver cancer with'middle-jiao deficiency and blood accumulation',which is considered to be the prominent feature of advanced liver cancer.'Middle-jiao deficiency and blood accumulation'refers to the pathological changes such as lack of yang-qi source,loss of emotional harmony,and difficulty in expelling evil spirits after deficiency of the lienogastric,leading to mixed cold and heat in the human body,disorder of qi and blood,formation of physical and solid evils such as blood stasis,water consumption,and poison,eventually leading to the emergence and progression of hepatoma.During the therapy,yin and yang spirits need to be distinguished,the spleen-stomach need to be protected,clearing out blockage of blood,dredging collaterals and draining turbidity from human body,in order to alleviate pain of the patients and prolong their life span.Emphasis and promotion on pathogenesis feature of'middle-jiao deficiency'provides an important idea for modern treatment of traditional Chinese medicine to deal with liver cancer.
