小陷胸汤加减治疗放射性肺炎痰热壅肺证经验探析

摘要
放射性肺炎(RP)是恶性肿瘤放疗的并发症,严重影响疾病预后及患者生存质量.通过长期临床观察放射线的致病特点,认为"外感温燥"为RP始动因素,"痰热壅肺"为病情进展的核心病机.在治疗RP痰热壅肺证时,以"清热涤痰"立法,以"小陷胸汤"为主方,以瓜蒌、半夏、黄连清热豁痰,并结合患者的证型及体质,配合调气、扶正、活血利水、散结等中药,在临床应用疗效显著.文章附应用小陷胸汤加减治疗RP医案一则,供临床参考.
Abstract
Radiation pneumonia(RP)is a complication of radiotherapy for malignant tumors,which seriously affects the prognosis of the disease and the quality of life of patients.Through long-term clinical observation of the pathogenic characteristics of radiation,it is believed that'exogenous heat and dryness'is the initiating factor of RP,'phlegm heat congesting the lung'is the core pathogenesis,which causes disease progressive.When treating pattern of phlegm-heat congesting the lung of RP,'heat clearing and phlegm removing'legislation is adopted,and'Xiaoxianxiong Decoction'is the main prescription.Trichosanthes,pinellia temata,and coptis rhizoma coptidis are used to clear heat and remove phlegm,combined with the patient's syndrome type and constitution,and combined with such traditional Chinese medicines as qi regulating,righting,promoting blood circulation and diuresis,and dispersing nodules,which has significant clinical effects.A medical case of Xiaoxianxiong Decoction in the treatment of RP is attached for clinical reference.
