"益髓醒神"法对卒中后失语工作记忆影响的思路探析

摘要
语言和记忆是人类大脑核心的认知功能,脑卒中的高致残率及复发率增加了卒中后语言、记忆等认知障碍的风险.卒中的发生多伴有语言、工作记忆相关的神经功能网络异常.文章从"益髓醒神"法阐述卒中后失语(PSA)记忆障碍病机与治疗中的"髓""神",以及益髓、醒神的关联,强调PSA记忆障碍与"脑髓空虚、窍闭不利、神失其守"及脑肾等脏腑功能协调关系密切.以"益髓醒神"法改善PSA患者的工作记忆为指标,结合现代研究,论述了EEG/ERP技术的工作记忆任务实验范式与神经心理测评相结合用于"益髓醒神"法对PSA干预效果的机制与评价应用,为PSA记忆障碍等非语言认知功能损伤提供理论指导,并为研究PSA的疗效机制提供新思路.
Abstract
Language and memory are the core cognitive functions of the human brain.At the level of cognitive neuropsychology,the high disability rate and recurrence rate of stroke increase the risk of cognitive impairment such as language and memory after stroke,and the occurrence of stroke is often accompanied by abnormalities of neural network related to language and working memory.This paper uses the method of'tonifying sui and arousing shen'to explain the relationship between marrow and spirit in the pathogenesis and treatment of post-stroke aphasia(PSA)and memory impairment,as well as the relationship between tonifying sui and arousing shen,emphasizing the relationship between PSA and memory impairment and'emptiness of the brain,badness closure of orifice,and loss of spirit,It is closely related to the coordination of viscera functions such as brain and kidney.Combined with modem research,using the method of'tonifying sui and arousing shen'to improve the working memory of PSA patients as an index,it discusses the combination of EEG/ERP technology's working memory task experimental paradigm and neuropsychological assessment for'tonifying sui and arousing shen'.The mechanism and evaluation application of the method on the effect PSA intervention,and discuss its practical significance,provide theoretical guidance for PSA memory impairment and other non-linguistic cognitive function impairment,in order to provide new ideas for the study of the therapeutic mechanism of PSA.
