叶天士动态治疫观探析
Exploration on YE Tianshi's dynamic view of epidemic management

摘要
清代著名温病学家叶天士,在疫病论治过程中多有造诣.其提出疫病之湿秽致病说,以卫气营血之表里四阶为病理发展阶段,湿阻阳郁为核心病机,随病程的不断深入,引发由郁渐瘀的病理状态改变.且叶天士的疫病治疗思想有明显的分阶段用药特征.邪在卫分,药以清透郁热;邪郁气分,药以芳透轻下分消;邪传营分,药以清解透转;邪人血分,药以散瘀贯阴.探析叶天士动态病机观可为当代疫病提供更为精准的辨治及选药依据,以提高临床疗效.
Abstract
YE Tianshi,a famous warm disease scientist in the Qing dynasty,had many attainments in the treatment of epidemic diseases.He put forward the theory of epidemic disease caused by dampness and obscenity,taking the four stages of development of Wei qi Ying and blood,and dampness blocking yang-depression as the core pathological mechanism,which triggered the change of pathological state from depression to stasis as the disease progressed.And YE Tianshi's idea of epidemic treatment has obvious characteristics of the use of drugs in stages.The evil in the Wei division,medicine to clear and penetrate the heat;the evil depresses the qi division,medicine to Fang penetrate the light under the subdivided elimination;the evil spreads to the Ying division,medicine to clear and penetrate;the evil into the blood division,medicine to disperse stasis through the yin.The analysis of YE Tianshi's dynamic view of disease mechanism can provide a more accurate basis for the identification and selection of medicine for contemporary epidemics,so as to improve clinical efficacy.
