目的:探究自拟三黄冰芒散在湿热痹阻型痛风性关节炎中的临床疗效.方法:将2020年12月-2022年6月重庆市北碚区中医院就诊的140例湿热痹阻型痛风性关节炎患者采用随机数表法分为试验组和对照组,各70例.对照组采用消痹凝胶贴膏外敷治疗,试验组采用自拟三黄冰芒散外敷治疗.治疗2周后,观察两组的疼痛视觉模拟评分(VAS)、中医证候评分、疗效、炎症因子水平[血尿酸(UA)、红细胞沉降率(ESR)、白细胞介素(IL)-1β及IL-6]及不良反应.结果:治疗后,两组VAS评分及中医证候评分均较治疗前显著降低(P＜0.05),且试验组均显著低于对照组(P＜0.05);试验组治疗前后VAS评分及中医证候评分差值均显著高于对照组(P＜0.05).试验组和对照组治疗总有效率分别为97.14％(68/70)和87.14％(61/70),试验组显著高于对照组(P＜0.05).治疗后,两组UA、ESR、IL-1β及IL-6均较治疗前显著降低(P＜0.05),试验组均显著低于对照组(P＜0.05).两组患者治疗期间均未出现皮肤瘙痒、疼痛等过敏反应.结论:自拟三黄冰芒散应用于湿热痹阻型痛风性关节炎治疗中,更有助于缓解患者的疼痛反应,抑制炎症反应,临床疗效优越且安全性良好.
Objective:To investigate the clinical efficacy of the self-developed Sanhuang Bingmang Powder in damp-heat impediment type of gouty arthritis.Methods:A total of 140 damp-heat impediment type of gouty arthritis patients in Chongqing Beibei Hospital of Traditional Chinese Medicine from December 2020 to June 2022 were randomly divided into test group and control group with 70 cases of each group.The control group was treated with Xiaobi gel plaster,and the test group was treated with self prepared Sanhuang Bingmang Powder.After 2 weeks of treatment,the visual pain score(VAS),TCM syndrome score,inflammatory factor level[blood uric acid(UA),erythrocyte sedimentation rate(ESR),interleukin-1 β(IL-1 β)And IL-6]and adverse reactions were observed.Results:After treatment,the VAS score and TCM syndrome score of the two groups were significantly lower than those before the treatment(P＜0.05),which in the test group was lower than those in the control group(P＜0.05).The difference of VAS score and TCM syndrome score before and after treatment in the test group was higher than that in the control group(P＜0.05).The total effective rates of the test group and the control group were 97.14％(68/70)and 87.14％(61/70),respectively,which were higher in the test group than those in the control group(P＜0.05).UA,ESR,IL-1 βand IL-6 levels were significantly reduced after treatment(P＜0.05),and they were lower in the test group than the control group(P＜0.05).No skin itching,pain and other allergic reactions occurred in both groups during treatment.Conclusion:The self prepared Sanhuang Bingmang Powder is applied to the treatment of gouty arthritis of damp heat arthralgia type,which is more helpful to alleviate the pain response of patients,inhibit inflammatory response,and has superior clinical efficacy and good safety.
