温肺方治疗嗜酸粒细胞性慢性鼻-鼻窦炎伴鼻息肉术后的临床疗效及机制

摘要
目的:观察温肺方治疗嗜酸粒细胞性慢性鼻-鼻窦炎伴鼻息肉术后临床疗效及对相关炎性因子白介素(IL)-13、IL-33、免疫球蛋白E(IgE)的影响.方法:92例肺气虚寒型ECRSwNP患者随机分为对照组和观察组,各46例.行鼻内镜手术后,对照组予西药糠酸莫米松鼻喷雾剂治疗,观察组在对照组基础上予自拟温肺方.治疗2个疗程后,观察治疗前后中医证候积分、VAS评分、鼻内镜积分、相关炎性因子含量变化及术后复发率.结果:治疗后,两组中医证候积分、VAS评分、鼻内镜积分、相关炎性因子水平较治疗前均显著下降(P＜0.05),且观察组显著优于对照组(P＜0.05).观察组复发率显著低于对照组(P＜0.05).结论:温肺方通过调节机体炎性因子水平,调整机体免疫功能,促进鼻腔黏膜及纤毛功能恢复,改善患者症状,降低ECRSwNP术后患者复发率.
Abstract
Objective:To observe the clinical efficacy of Wenfei Prescription in the treatment of eosinophilic chronic rhinosinusitis with nasal polyps after operation and its effect on related inflammatory factors interleukin(IL)-13,IL-33 and immunoglobulin E(IgE).Methods:A total of 92 patients with ECRSwNP of lung qi deficiency and cold type were randomly divided into control group and observation group,with 46 cases in each group.After endoscopic sinus surgery,the control group was treated with mometasone furoate nasal spray.The observation group was given self-made Wenfei Prescription on the basis of the control group.After 2 courses of treatment,the changes of TCM syndrome score,VAS score,nasal endoscopy score,related inflammatory factor content and postoperative recurrence rate were observed before and after treatment.Results:After treatment,the TCM syndrome score,VAS score,nasal endoscopy score and related inflammatory factor levels in two groups were significantly lower than those before treatment(P＜0.05),and the observation group was better than the control group(P＜0.05).The recurrence rate in the observation group was significantly lower than that in the control group(P＜0.05).Conclusion:Wenfei Prescription can promote the recovery of nasal mucosa and cilia function,improve the symptoms of patients and reduce the recurrence rate of patients after ECRSwNP by regulating the level of inflammatory factors and adjusting the immune function of the body.
