清脂消岩方干预绝经后乳腺癌芳香化酶抑制剂治疗伴高脂血症的临床疗效观察

摘要
目的:观察清脂消岩方干预绝经后乳腺癌患者芳香化酶抑制剂治疗伴高脂血症的临床疗效水平及安全性.方法:采用前瞻随机对照方法,将乳腺癌芳香化酶抑制剂治疗伴高脂血症64例患者分为对照组、治疗组各32例,对照组予芳香化酶抑制剂、阿托伐他汀钙片治疗,治疗组在对照组基础上加用清脂消岩方,两组均治疗12周.治疗前后观察血脂、中医证候评分、肿瘤指标、安全性指标.结果:治疗组血脂治疗总有效率93.75％(30/32),显著优于对照组84.38％(27/32)(P＜0.05).与治疗前比较,两组治疗后均能显著降低甘油三酯(TG)、总胆固醇(TC)、低密度脂蛋白胆固醇(LDL-C)、载脂蛋白B(APo-B)水平(P＜0.01),治疗组能升高高密度脂蛋白胆固醇(HDL-C)水平(P＜0.05).且治疗后,治疗组TC、LDL-C、APo-B水平显著低于对照组(P＜0.01,P＜0.05).在中医证候方面,治疗组总有效率93.75％(30/32),显著高于对照组总有效率68.75％(22/32)(P＜0.01).与治疗前比较,两组治疗后均显著降低中医证候积分值(P＜0.01),且治疗组显著低于对照组(P＜0.01).治疗后,治疗组患者糖类抗原153较前显著下降(P＜0.01).两组患者均未出现心肝肾功能损害.结论:清脂消岩方能改善绝经后乳腺癌芳香化酶抑制剂治疗伴高脂血症患者的血脂水平和中医临床症状.
Abstract
Objective:To observe the clinical efficacy and safety of Qingzhi Xiaoyan Recipe in the treatment of postmenopausal breast cancer patients with hyperlipidemia by aromatase inhibitor therapy.Methods:A prospective randomized controlled trial was used,64 patients with postmenopausal breast cancer and hyperlipidemia treated with aromatase inhibition were randomized into a control group and a treatment group,each consisting of 32 patients.The control group received aromatase inhibitor and Atorvastatin Calcium Tablets.The treatment group was added with Qingzhi Xiaoyan Recipe on the basis of the control group.Both groups were treated for 12 weeks.Blood lipids,traditional Chinese medicine(TCM)symptom scores,tumor indicators,and safety indicators were observed before and after treatment.Results:The effective rate of lipid treatment in the treatment group was 93.75％(30/32),significantly better than that in the control group of 84.38％(27/32)(P＜0.05).Compared with before treatment,both groups were able to significantly reduce triglyceride(TG),total cholesterol(TC),low-density lipoprotein cholesterol(LDL-C),and apolipoprotein-B(APo-B)levels after treatment(P＜0.01),while the treatment group was able to increase high-density lipoprotein cholesterol(HDL-C)levels(P＜0.05).After treatment,the levels of TC,LDL-C,and APo-B in the treatment group were significantly lower than those in the control group(P＜0.01,P＜0.05).In terms of TCM syndrome,the total effective rate of the treatment group was 93.75％(30/32),significantly higher than the total effective rate of the control group of 68.75％(22/32)(P＜0.01).Compared with before treatment,both groups significantly reduced the TCM syndrome score after treatment(P＜0.01),and the treatment group was significantly lower than the control group(P＜0.01).After treatment,the carbohydrate antigen 153 levels in the treatment group significantly decreased compared to before(P＜0.01).Both groups of patients did not experience any impairment of heart,liver,or kidney function.Conclusion:Qingzhi Xiaoyan Recipe can improve the blood lipid level and TCM clinical symptoms of postmenopausal breast cancer patients with hyperlipidemia treated with aromatase inhibitor.
