摘要
铁死亡为新发现的细胞死亡形式,大量研究表明,将铁死亡作为肺癌治疗中的靶点,潜力巨大."阴阳气不相顺接"出自《伤寒论》,李忠教授提出肺癌发生发展的病机基础为"阴阳气不相顺接";正气耗损为肺癌进展的内在因素,是关键;内生癌毒为肺癌病机的外在表现,是直接影响因素.肺癌的铁死亡机制与"阴阳气不相顺接"相互贯通.铁死亡在肺癌进展中同时具有促瘤与抑瘤的调控作用.正气耗散无法诱导肺恶性肿瘤细胞铁死亡,抑瘤作用减弱;有形癌毒诱导肺恶性肿瘤细胞铁死亡的促瘤作用增强.正气耗散与内生癌毒相合,使肺恶性肿瘤细胞铁死亡朝不利方向发展,导致肺癌进展.因此,在治疗中在辨证施治基础上重视固摄法,固摄正气及癌毒尤为关键.
Abstract
Iron death is a newly discovered form of cell death,and numerous studies have shown that using iron death as a target in the treatment of lung cancer has great potential.The concept of'yin qi and yang qi circulating out of order'comes from Treatise on Febrile Diseases.Professor LI Zhong proposed that the pathogenesis of lung cancer is based on the concept of'yin qi and yang qi circulating out of order'.The depletion of health qi is an intrinsic factor in the progression of lung cancer and is crucial;endogenous cancer toxin is an external manifestation of lung cancer pathogenesis and a direct influencing factor.The mechanism of iron death in lung cancer is interconnected with the phenomenon of'yin qi and yang qi circulating out of order'.Iron death has both pro tumor and anti tumor regulatory effects in the progression of lung cancer.The dissipation of health qi cannot induce ferroptosis in lung malignant tumor cells,and its anti-tumor effect is weakened.The tumor promoting effect of tangible cancer toxin on inducing ferroptosis in lung malignant tumor cells is enhanced.The dissipation of health qi is combined with endogenous cancer toxins,causing iron death in lung cancer cells to develop in an unfavorable direction,leading to the progression of lung cancer.Therefore,in the treatment,it is crucial to attach importance to the consolidation method of strengthening qi and cancer toxin based on syndrome differentiation and treatment.
