隔药灸脐法调控结肠组织TRPV1离子通道对腹泻型肠易激综合征大鼠的作用机制

目的:通过观察隔药灸脐法对腹泻型肠易激综合征(IBS-D)大鼠腹泻症状、内脏敏感性及结肠组织中瞬时受体电位香草酸受体亚型1(TRPV1)、P物质(SP)、降钙素基因相关肽(CGRP)的影响,探讨其疗效及作用机制.方法:将大鼠随机分为对照组、模型组、隔药灸脐组、隔药灸脐+TRPV1拮抗剂组.采用乙酸灌肠联合慢性捆绑束缚刺激建立IBS-D大鼠模型.治疗结束后观察各组大鼠腹泻症状、腹壁撤退反射(AWR)评分;采用免疫组化、Western Blot、实时荧光定量PCR技术检测各组大鼠结肠组织中TRPV1、SP、CGRP蛋白及mRNA表达量.结果:与对照组比较,模型组大鼠腹泻症状、AWR评分以及结肠组织中TRPV1、SP、CGRP蛋白及mRNA表达量显著升高(P<0.01);与模型组比较,经隔药灸脐治疗后上述指标均显著改善(P<0.01);且隔药灸脐组疗效优于隔药灸脐+TRPV1拮抗剂组(P<0.01,P<0.05).结论:隔药灸脐法可以改善IBS-D大鼠腹泻症状,下调结肠组织中TRPV1、SP、CGRP表达,有效缓解内脏高敏感性,其机制可能与TRPV1离子通道有关.
Mechanism of regulating TRPV1 ion channel in colon tissue by herb-partitioned moxibustion on the navel in diarrhea-predominant irritable bowel syndrome rats
Objective:To observe the effects of herb-partitioned moxibustion on the navel in diarrhea symptoms,visceral sensitivity index and TRPV1,SP and CGRP in colon tissue of diarrhea-predominant irritable bowel syndrome(IBS-D)rats to explore its curative effect and mechanism.Methods:The rats were randomly divided into control group,model group,herb-partitioned moxibustion navel group,herb-partitioned moxibustion navel+TRPV 1 antagonist group.After treatment,the diarrhea symptoms of rats in each group were observed,abdominal wall withdrawal reflex score(AWR),the expression of TRPV1,SP,CGRP protein and mRNA in the colon of rats in each group were detected by enzyme-linked immunoassay and immunohistochemistry,Western Blot,and real-time fluorescent quantitative PCR technology.Results:Compared with the control group,diarrhea symptoms,AWR score and the protein and mRNA expressions of TRPV1,SP and CGRP in colon tissue of rats in model group were significantly increased(P<0.01).Compared with model group,the above indexes were significantly improved after herb-partitioned moxibustion on the navel(P<0.01).The curative effect of herb-partitioned moxibustion on the navel was better than that of herb-partitioned moxibustion on the navel+TRPV1 antagonist group(P<0.01,P<0.05).Conclusion:Herb-partitioned moxibustion on the navel can effectively improve diarrhea symptoms in rats with IBS-D,down-regulate the expression levels of TRPV1,SP and CGRP in colon tissue,and effectively alleviate visceral hypersensitivity.The mechanism may be related to TRPV1 ion channel.
