二补助育汤调控子宫内膜容受性的临床和基础研究

随着女性生育需求的增加及辅助生殖技术的广泛应用,如何调控子宫内膜容受性成为生殖领域亟待解决的关键问题.国医大师肖承惊教授在改善子宫内膜容受性方面,独创"补肾气,通胞脉"之"二补助育汤".临床研究结果显示二补助育汤及其改良方能改善中医证候、激素水平,增加内膜血流,改善内膜微环境,纠正血液高凝状态,促进内膜血管网建立与稳定,进而调控子宫内膜容受性.效应机制涵盖干细胞因子、整合素αVβ 3、同源盒基因10、自噬、血管生成及氧化应激等多个靶点及相关信号通路,通过作用于胚胎着床过程中定位、黏附、侵入3个环节,对子宫内膜容受性产生调控作用.希冀为系统、科学地传承发扬燕京萧氏妇科的学术经验,提高临床妊娠率,助力中医药解决辅助生殖领域的难点问题,提供理论及实验依据.
Clinical and basic research of regulating endometrial receptivity by Erbu Zhuyu Decoction
With the increasing demand for female fertility and the wide application of assisted reproduction technology,how to regulate endometrial receptivity has become a key issue in the field of reproduction that needs to be resolved urgently.In improving the endometrial receptivity,professor XIAO Chengcong,master of Chinese medicine originally creates the Erbu Zhuyu Decoction,which is designed to'replenish the kidney qi and open the cytosolic veins'.Clinical studies have shown that Erbu Zhuyu Decoction and its improved formula can improve Chinese medicine symptoms and hormone levels,increase endometrial blood flow,improve the endometrial microenvironment,correct the hypercoagulable state of the blood,promote the establishment and stability of the endometrial vascular network,and then regulate the endometrial receptivity.The mechanism of effect covers multiple targets and signalling pathways such as stem cell factor,integrin α V β 3,homobox gene 10,autophagy,angiogenesis,and oxidative stress,which regulate endometrial receptivity through the three links of localisation,adherence,and invasion of the follicle during the process of follicle implantation.We hope to provide theoretical and experimental basis for systematically and scientifically inheriting and developing the academic experience of Yanjing XIAOs'academic school of gynecology,improving the clinical pregnancy rate,and helping traditional Chinese medicine to solve the difficult problems in the field of assisted reproduction.
