针刺干预炎症性肠病与情绪障碍共病的机制探究

摘要
炎症性肠病(IBD)是一组慢性、复发性肠道炎症疾病,约1/3的患者同时伴有情绪障碍,情绪障碍对IBD患者疾病活动度、病程和预后有直接影响,这可能与肠道中的微生物构成和代谢影响免疫系统、中枢神经系统的成熟和发育有关.针刺对炎症性肠病与情绪障碍共病具有良好疗效,其内在干预机制目前尚未完全明确,目前认为针灸可通过调节肠道菌群、下丘脑-垂体-肾上腺轴、自主神经及迷走神经从而起到治疗作用,文章尝试从以上机制着手,探讨针刺治疗IBD与情绪障碍共病的可能途径.
Abstract
Inflammatory bowel disease(IBD)is a group of chronic and recurrent intestinal inflammatory disorders.Approximately one third of patients with IBD also experience mood disorders,which have a direct impact on disease activity,course,and prognosis.This may be related to the effects of microbial composition and metabolism in the gut on the maturation and development of the immune and central nervous systems.Acupuncture has shown good efficacy for the comorbidity of IBD and mood disorders,and its underlying mechanisms of action are not fully understood.It is currently believed that acupuncture may achieve therapeutic effects by regulating the gut microbiota,hypothalamic-pituitary-adrenal(HPA)axis,autonomic nervous system(ANS),and vagus nerve(VN).This article attempts to explore the potential pathways for acupuncture in treating the comorbidity of IBD and mood disorders from the aforementioned mechanisms.
