艾灸原发性痛经患者热敏态三阴交穴即刻效应的局部一致性与功能连接研究

中文摘要：目的:采用静息态功能磁共振技术(rs-fMRI),通过局部一致性(ReHo)和功能连接(FC)分析,探讨艾灸原发性痛经(PD)患者热敏态穴位的即刻中枢响应机制.方法:选取公开招募的20例经过热敏探查三阴交穴具有热敏灸感的PD患者作为研究对象.采集艾灸前后的rs-fMRI数据,进行ReHo分析和以脑岛为种子点的FC分析并比较艾灸前后差异.数据处理和统计分析采用MATLAB 2013b平台的RESTplus 1.2和SPM 12.结果:艾灸热敏态三阴交穴后辅助运动区的ReHo减低.与左脑岛的FC减低的脑区有中央旁小叶、中央前回、辅助运动区、中央后回.与右脑岛的FC减低的脑区有距状裂周围皮层、舌回、枕中回、枕下回、枕上回、中央旁小叶、辅助运动区、中央后回、中央前回.结论:艾灸PD患者热敏态三阴交穴产生了显著的即刻效应,辅助运动区局部一致性、SMN-脑岛功能连接、枕叶-脑岛功能连接的改变可能是其重要中枢机制.

外文摘要：Objective:To explore the immediate central response mechanism of heat-sensitive acupoints in patients with primary dysmenorrhea(PD)by using resting state functional magnetic resonance imaging(rs-fMRI),through the analysis of regional homogeneity(ReHo)and functional connectivity(FC).Methods:A total of 20 PD patients with heat-sensitive moxibustion sensation after thermal exploration at Sanyinjiao acupoint(SP6)were selected as the research objects.The rs-fMRI data before and after moxibustion were collected,and the ReHo analysis and FC analysis with the insula as the seed point were performed to compare the differences before and after moxibustion.RESTplus 1.2 and SPM 12 on MATLAB 2013b platform were used for data processing and statistical analysis.Results:After moxibustion at heat-sensitive Sanyinjiao acupoint(SP6),the ReHo of the supplementary motor area decreased.The brain areas with reduced FC with the left insula include the paracentral lobule,precentral gyrus,supplementary motor area,and postcentral gyrus.The brain areas with reduced FC with the right insula include cortex around the distance fissure,lingual gyrus,middle occipital gyrus,inferior occipital gyrus,superior occipital gyrus,paracentral lobule,supplementary motor area,postcentral gyrus,and precentral gyrus.Conclusion:Moxibustion at heat-sensitive Sanyinjiao(SP6)in PD patients produced significant immediate effects,and changes in regional homogeneity of the supplementary motor area,SMN-insula functional connectivity,and occipital lobe-insula functional connectivity may be the important central mechanisms.
