目的:探索针刺治疗对不同烟草依赖程度吸烟者戒断效果的影响.方法:将1 094例烟草依赖受试者根据法氏烟草依赖评估量表(FTND)评分分为轻度、中度、重度3组,均采用针刺干预,穴位选择为百会、印堂、列缺、合谷、内关、三阴交、太冲和足三里.每次治疗20 min,每周2次,持续8周.比较3组受试者第4、8周点戒烟率、治疗前后的每日吸烟量及呼出气体中的一氧化碳(CO)含量和安全性评价.结果:3组受试者第4、8周轻度烟草依赖组点戒烟率分别为63.3％(219/346)、65.6％(227/346),显著高于同期中度[44.2％(205/464)、46.8％(217/464)]和重度[39.4％(112/284)、39.8％(113/284)]烟草依赖组(P＜0.01).各组治疗后每日吸烟量较治疗前均减少(P＜0.01),治疗后3组间比较差异有统计学意义(P＜0.01).治疗后呼气中CO含量较治疗前均降低(P＜0.01),3组间比较差异有统计学意义(P＜0.01),3组内两两比较,轻度与中度、重度差异有统计学意义(P＜0.01).结论:针刺治疗对烟草依赖程度越轻的受试者戒断效果越好.
Retrospective analysis of effects of acupuncture on the abstinence of smokers with different degree of dependence based on real world
Objective:To explore the effects of acupuncture on the abstinence of smokers with different degree of tobacco dependence.Methods:A total of 1 094 tobacco dependent subjects were divided into mild,moderate and severe groups according to Fagerström test for nicotine dependence(FTND).Acupuncture was used in all three groups,and the acpoints were selected as Baihui(DU 20),Yintang(EX-HN 3),Lieque(LU 7),Hegu(LI 4),Neiguan(PC 6),Sanyinjiao(SP 6),Taichong(LR 3)and Zusanli(ST 36).Each treatment was 20 min,twice a week,and continued for 8 weeks.The smoking cessation rate at week 4 and week 8,the daily smoking quantity before and after treatment,the carbon monoxide(CO)content in exhaled gas and the safety evaluation were compared among the three groups.Results:At week 4 and week 8,the smoking cessation rates of the mild group were 63.3％(219/346)and 65.6％(227/346),which were higher than the moderate group[44.2％(205/464)and 46.8％(217/464)],and the severe group[39.4％(112/284)and 39.8％(113/284)](P＜0.01).The daily smoking amount of all groups after treatment was decreased compared with that before treatment(P＜0.01),and there was statistical significance among 3 groups after treatment(P＜0.01).The CO content in expiratory breath after treatment was lower than that before treatment(P＜0.01),the difference among 3 groups was statistically significant(P＜0.01),and pairwise comparison among the three groups showed statistically significant differences between mild and moderate and severe(P＜0.01).Conclusion:Acupuncture treatment has different effects on different tobacco dependent subjects.The lighter the degree of tobacco dependence,the better the effect of acupuncture withdrawal.
