"舒筋活络"推拿法对骨关节炎固有免疫应答的调控作用

目的:探讨"舒筋活络"推拿法对骨关节炎(OA)相关固有免疫应答的调控作用.方法:将30只大鼠随机分为正常组、推拿组和模型组,每组10只.采用前交叉韧带横断术构建OA模型,4周后推拿组行"舒筋活络"推拿手法,隔日1次,共计4周;正常组和模型组不予干预.采用HE染色对滑膜组织进行组织学观察;采用RT-qPCR和Western Blot对固有免疫中炎性途径的重要元件TLR4、TRAF6、NF-κB的mRNA相对表达量和蛋白水平进行分析.结果:与正常组比较,模型组的大鼠滑膜组织衬里层明显增厚,滑膜细胞聚集、增多、排列紊乱,有血管翳生成,滑膜组织中TLR4、TRAF6、NF-κB的mRNA和蛋白表达水平均显著上调(P＜0.01);与模型组比较,推拿组滑膜衬里层厚度、细胞排列、炎性浸润和血管翳形成等情况均减轻,TLR4、TRAF6、NF-κB的mRNA和蛋白表达水平均显著下调(P＜0.05,P＜0.01).结论:"舒筋活络"推拿法能有效减轻滑膜增殖和炎症细胞浸润;可通过抑制Toll样受体介导的固有免疫应答中相关途径发挥治疗OA的作用.
Abstract
Objective:To explore the regulatory effect of 'Shujin Huoluo'massage therapy on the innate immune response in osteoarthritis(OA).Methods:Thirty rats were randomly divided into normal group,massage group and model group,with ten in each.Anterior cruciate ligament transection was used to construct OA model.The massage group received'Shujin Huoluo'massage therapy once every other day for 4 weeks.The normal and model groups received no intervention.4 weeks later,synovial tissues were histologically examined by HE staining.The relative mRNA expression and protein levels of TLR4,TRAF6,and NF-κB,key components of the inflammatory pathway in innate immunity,were analyzed by RT-qPCR and Western Blot.Results:Compared with normal group,the model group displayed a significant thickening of the synovial lining,with synovial cells accumulating,increasing in number,and becoming disorganized,alongside pannus formation.The mRNA and protein expression levels of TLR4,TRAF6,and NF-κB in the synovial tissue were significantly upregulated(P＜0.01).In comparison to the model group,the massage group showed reductions in synovial lining thickness,improved cell arrangement,decreased inflammatory infiltration,and less pannus formation.Furthermore,the mRNA and protein expression levels of TLR4,TRAF6,and NF-κB were significantly decreased(P＜0.05,P＜0.01).Conclusion:'Shujin Huoluo'massage therapy can effectively reduce synovial proliferation and inflammatory cell infiltration.It demonstrates potential therapeutic effects by inhibiting pathways involved in Toll-like receptor-mediated innate immune responses.
