摘要
痹的本质为不通,痿的本质为功能下降,强调痹、痿属病机概念,根据痹、痿相关性凝练提出"因痹致痿"病机及通痹治痿法.以该病机治法为视角解析张子和治疗痹、痿医案,认为汗、吐、下攻邪三法具有以攻为补、寓补于通特点.汗、吐、下攻邪三法祛邪通痹,流通气血治痿.三法是通痹的手段,通痹是三法的直接作用,治痿是三法的根本目的,结合张子和四则治疗痹、痿医案,具体阐释祛邪通痹治痿:骨痹致痿,汗法熨蒸散外邪阻滞,针药并用祛内传寒湿;风寒湿痹致痿,下法汗法次第并用,祛邪通痹;水痹致痿,峻下泄水,淡渗利水,峻缓结合,祛邪通痹治痿;痿病日久,痿、痹并存,仍可以通为补,通泻阳明.
Abstract
The essence of Bi disease is impassability,and the essence of flaccidity is function decline.It emphasizes that Bi disease and flaccidity belong to the pathogenesis,according to the correlation between Bi disease and flaccidity,we concisely put forward the pathogenesis of'flaccidity due to Bi disease'and the method of treating flaccidity by Tongbi.From the perspective of this pathogenesis and treatment method,analyzing ZHANG Zihe's medical case of treating Bi and flaccidity disease,it is believed that the three methods of attacking evil of ZHANG Zihe have the characteristics of taking attack as tonifying and taking circulation as tonifying.Perspiration emetic therapy and purgation three methods to dispel evil and Tongbi,circulate qi and blood,and treat flaccidity.Three methods are the means to Tongbi,which is the direct effect of the three methods,and the fundamental purpose of the three methods is to treat flaccidity,in combination with ZHANG Zihe's four medical cases of treating Bi and flaccidity disease,the article specifically explains the treatment of flaccidity with dispel evil and Tongbi.Bi disease of bone causes flaccidity,perspiration irons and dissipates external evil block,acupuncture and medicine are used to dispel internal cold dampness;Bi disease of wind cold and dampness evil causes flaccidity,the purgative remedy and method to induce perspiration are used in turn to dispel evil factors and Tongbi;Bi disease of water causes flaccidity,drags water under precipitous conditions,makes water easy to seep,combines precipitousness with gentleness,and dispeling flaccidity by dispeling evil and Tongbi;Flaccidity disease for a long time,causing flaccidity and Bi disease coexist,and can still tonify by improving circulation,dispels evil and promotes circulation by circulating Yangming.
